Position Summary : This position is responsible for providing outpatient rehabilitation services within a home-based setting, focusing on the functional needs of the older adult population.

The in-home setting could be a patient's private residence or in a patient's room in a Dementia-care facility, in an assisted living community, or in an independent living community.

This position will evaluate and treat in the most functional environment which is the patient's home and assess the need for, develop, and deliver occupational therapy interventions to facilitate rehabilitation both on an intermittent and maintenance-based scale.

The Occupational Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients.

The Occupational Therapist will implement individualized therapy programs designed to restore, reinforce, and enhance performance and optimize independence.

In addition, this position instructs, educates and trains patients and caregivers in those skills and functions essential in promoting quality, independence, and productivity, in order to diminish or correct movement disorder pathology.

Essential Functions :

Works closely with the Regional Director of Health & Wellness as well as the Director of Business Development to ensure proper execution and maintenance of the Health and Wellness Programs

Screens, examines and evaluates patients, including history and systems review

Provides thorough HEP education to patients and caregivers in a manner they can understand, and plans appropriately for discharge when medical necessity no longer indicates skilled therapeutic care.

Makes clinically necessary referrals to other disciplines as well as other agencies in order to address the most complex and comprehensive needs of the patient.

Consults with other members of the Health & Wellness team to select the most appropriate therapeutic regimen consistent with the objective findings, functional needs and capabilities of each patient.

Adapts program and / or plan of care according to the needs of individual patient.

Orders supplies and equipment (DME) as necessary; trains patient and caregivers on how to use equipment.

Maintains, identifies, and reports the need for repair of equipment as necessary

High quality patient care. Ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.

Participates in departmental initiatives and demonstrates effective communication with all team members.

Facilitates professional growth, development and overall improvement of departmental performance.

Performs other professional duties associated with the care of the patient, development of discipline-specific therapy services, and design of program delivery.

Application of appropriate tests, measures (FOMs) and therapeutic intervention.

Identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions / contraindications and design and plan of care.

Follows proper continuing education requirements and assures up to date licensure.

Services include but are not limited to application of therapeutic techniques in areas such as :

Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuro-motor development training, relaxation, strength, power, and endurance training

Manual therapy

Airway clearance techniques

Integumentary repair and protection

Use of electrotherapeutic, physical agent and mechanical modalities

Education on Chronic Disease Management

Falls Assessment and Interventions to reduce fall risk

Case Management in the home and community

Qualifications :

Minimum Qualifications

Valid State Occupational Therapy License REQUIRED to start

Cardio-Pulmonary Resuscitation (CPR) certification required

Preferred Qualifications

Dementia experience

Geriatric experience

Interest and / or experience in lymphedema

At least 1 year of geriatric experience

POSITION SUMMARY :

The occupational therapist will perform clinical duties including evaluations, formulating a treatment plan and ensuring that all aspects of the treatment plan are carried out concurrent with the medical protocols established by Concentra Health Services.

The occupational therapist will coordinate with the Center Therapy Director to perform business and financial duties directly related to the provision of therapy services at the center.

Responsibilities

THE DETAILS :

  • Ensure delivery of high quality patient care services
  • Perform direct patient care, including evaluation of muscle, skeletal, neurological and other physical problems.
  • Support and encourage multidisciplinary coordination of treatment.
  • Ensure proper documentation of patient care.
  • Maintain professional standards through compliance with quality assurance policies / procedures as well as applicable regulations and corporate policies.
  • Implement effective communication with own and other departments' staff, with medical staff and patients to ensure coordinated efforts and provision of high quality service and optimal patient care.
  • Demonstrate effective problem-solving and conflict-resolution abilities
  • Inform Therapy Director of department problems, needs and solutions in a timely manner
  • Plan, design, implement and interpret specific physical therapy treatment programs for patients to facilitate the patient's recovery and attainment of maximum physical potential
  • Educate patients in the proper care and use of supports and performance of exercise programs.
  • Perform or delegate patient treatment to include modalities, therapeutic exercises, gait training and hydrotherapy.
  • Follow appropriate documentation and reporting procedures.
  • Secure prescription for each patient treated.
  • Work closely with the physician to ensure comprehensive delivery of services and quality patient care.
  • Assist marketing staff with tours as necessary.
  • Take all necessary precautions to protect center assets and property against loss or waste through negligence or dishonesty.

Qualifications

EDUCATION / CREDENTIALS :

  • Master’s Degree in Occupational Therapy from an accredited college / university
  • Current state licensure as an Occupational Therapist, Hand Therapy Certification preferred but not required
  • Successful completion of the National Registry Examination
  • Valid BLS

JOB-RELATED EXPERIENCE :

  • Customarily has at least two or more years of demonstrated clinical experience with hand, wrist and elbow rehabilitation

OT Job Summary :

The Occupational Therapist provides quality care to all residents. The OT evaluates patients, plans, and implements patient treatment, plans, and coordinates patient discharge, and prepares detailed progress reports and appropriate patient documentation.

Treatments will include training in daily living activities, functional skills, motor development, compensation techniques use of adaptive equipment and splinting.

The Occupational Therapist will also test and evaluate patients’ physical and mental abilities analyzing rehabilitation goals and consult with the rehab team in order to coordinate the most effective occupational therapy program.

Qualifications :

Licensed as an Occupational Therapist in the State of California.

  • Must have valid CPR certification.
  • Must have a completed physical examination from a licensed physician.
  • Must have a negative TB skin test within the past year OR a clear Chest X-ray within the past five years.

Responsibilities and Duties :

Evaluate patients promptly upon receiving physician referral for treatment.

  • Develop effective treatment plans.
  • Obtain approval for services from referring physician.
  • Treat patients according to treatment plan approved by physician.
  • Record evaluations, daily treatment notes, weekly progress notes, 14-day progress report to the physician, recertifications, and discharge summaries per Company and facility policy and procedure
  • Participate in discharge planning.
  • Submit all billing logs and necessary information on a timely basis.
  • Able to supervise up to two Certified Occupational Therapy Assistants.
  • Supervise students during the internship.
  • Provide clearly defined, career-related (i.e., non-clerical) responsibilities and tasks in accordance with the students’ academic programs.
  • Inform student interns about ensuring their compliance with company, facility, state, and federal guidelines.

  • May be entry level or experienced.
  • Must have strong computer skills, including proficiency with Microsoft Office.
