In this position you will play a key role in improving the health of our residents by monitoring their chronic conditions.
Daily tasks include a combination of clerical and clinical duties :
Clinical
- Taking residents’ vital signs, ensuring they are captured in our software
- Complete software guided individualized patient health screener questionnaires to support the creation of a care plan.
Monitoring and documenting residents’ symptoms for physicians review during clinical exams and consultations
Frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes.
Administrative
Documenting residents’ medical information, including medical history and insurance information
Perform data entry tasks to document resident records within facility data bases and resident files.
Obtaining consent forms when needed
Requirements
Certified as a Medical Assistant within the last 2 years
The ideal candidate will have the desire to help Assisted Living residents with their medical care
Fully vaccinated per DOH requirements
Pay : $15.00 - $20.00 per active patient per month (Approx 80 patients)

Set your own schedule, as long as monthly tasks are completed

Position Summary
Under direction and in accordance with established policies and procedures, assists with planning, implementing and evaluating patient care This role is the primary resource for the team of medical assistants, responding to staff inquiries for guidance on policy and procedures This role also assists in the assignment of work distribution to staff and the follow through with completion of these tasks, as well as is responsible for overseeing the smooth function of patient flow.
Job Responsibilities
- Supports and provides assistance in the training and continuing development of staff.
- Maintains inventory of supplies, equipment and / or reagents. May order supplies / equipment / reagents as authorized or ensure that needs are escalated appropriately.
Receives supply orders and confirms accuracy of delivery. May assist with cost control.
- Prepares exam rooms prior to patient visit. Cleans rooms and instruments and re-stocks supplies between patients.
- Answers phones, responds to inquiries and triages calls to appropriate personnel.
- Fulfills patient care responsibilities which may include, taking vital signs, updating patient history, collecting blood, urine and stool specimens and assisting providers / nurses in the exam room.
Inputs patient data into the practice management system.
- Provides recommendations to improve work flow, communication and overall protocols in an effort to better serve the needs of patients and the department.
- May assist with educating patients on specialty-specific procedures and after-care techniques or protocols.
- Greets registered patients at time of visit and triages patients through the patient flow process. Checks schedules and accompanies patients to exam / procedure room.
Assists patients as needed with walking, transfers, dressing and preparing for exam.
- Fulfills clerical responsibilities which may include, sending / receiving / scanning medical records, obtaining diagnostic reports, hospital notes & referral information, completing forms / requisitions, managing the charting process & sending specimens to labs.
- Responsible for maintaining a sterile environment. Ensures standard infection control precautions are observed.
- Trains and manages the orientation of new staff.
- Assigns work to staff and ensures completion.
- Monitors and ensures clinical equipment is functioning properly. Escalates issues to supervisor.
- Regularly checks dated materials and medications for expiration date and discards expired items.
Education
High School Diploma
Experience
Approximately 2 years of related experience, preferably in relevant specialty area.
Completion of a medical assistant training course that includes phlebotomy training is highly desired.
Candidates with extensive medical assistant work experience, along with phlebotomy experience, may be considered.
If required to draw blood, proper phlebotomy training is required prior to performing those duties.
Knowledge, Skills and Abilities
- Demonstrated strong communication and interpersonal skills; demonstrated ability to interact with multiple constituencies and exercise "people skills".
- Demonstrates ability to exercise standards of professionalism, including appearance, presentation and demeanor.

Responsibilities :
Direct patient care and support
- Perform nursing tasks under the direction of the registered nurse or authorized health care providers
- Interview patients and records medical history, take vital signs, and prepare patients for medical visits

Responsibilities
