JOB SUMMARY

Under the direction of the Director of Plant Operations & Maintenance installs, maintains and repairs equipment and building systems in accordance with manufacturers specifications, hospital policy and all applicable codes.

JOB QUALIFICATIONS

  • High school diploma, or equivalent required, Trade School
  • Licensed by the City of Chicago as a Supervisory Electrician
  • 1 to 2 years supervisory experience of other workers as required to complete various assignments
  • Ability to read and understand blueprints and schematics, this requirement will be essential in the installation, operation and repair of various equipment that the Electrician will be required to work on
  • Must be in good physical condition as this position requires working as part of a construction department and walking, climbing ladders, and carrying equipment and tools is required.

SUPERVISION

Exercised : May be called upon to indirectly supervise 1 to 4 Maintenance Workers / Remodlers on various day-to-day projects or work that has been assigned as this relates to the installation, repair, or operation of the Electrical Systems and equipment in the Hospital

Received : Reports directly to the Director, Plant Operations & Maintenance. Works with a significant degree of independence.

Supervisory contact will generally consist of work review, problem review, and priority setting sessions.

JOB RELATIONSHIPS

Will be required to interact with staff and visitors to determine needs for the services provided.

Interacts with vendors and Outside Contractors to bid projects and services. Will be required to inspect and coordinate vendor’s work.

JOB DUTIES

  • Demonstrates the WMH Transformation for Success eight elements, which are key in providing quality service to patients and customers.
  • Operates, maintains, repairs & installs all equipment as assigned in accordance with guidelines, building codes and departmental policies
  • Completes all preventive maintenance assigned within the guidelines of departmental policies.
  • Keeps all logs, records and other reports as assigned on the building and up to date data on all electrical systems and components of these systems.
  • Insures that all testing and inspection is performed in accordance with Codes and requirements of the various agencies regulating Hospitals and Health Care Institutions
  • Responds to all calls given in a timely manner by completing the request or communicating with the requestor conditions causing a delay
  • Does material take-offs, ordering, and inventorying of supplies for daily needs and major projects.
  • Responsible for obtaining and maintaining the Electrical Maintenance Permit issued for the Hospital by the City of Chicago.
  • Keeps all assigned areas neat clean and in good repair at all times

Job Summary

Provides advanced / specialized occupational therapy services appropriate to patients’ needs, utilizing necessary resources to ensure optimal patient care.

Provides training and guidance to the Occupational Therapy team.

Job Duties

Demonstrates advanced skills in evaluating, interpreting, planning, educating, and implementing age-appropriate and functional occupational therapy treatment for patients.

Is responsible for using specialty skills in specific practice areas when applicable in addition to performing staff OT duties including but not limited to assessment of underlying nerve, muscle, joint integrity to support functional activity and provision of complex treatments utilizing modalities, therapeutic exercise, manual therapy, neuromuscular re-education, ADL training / use of adaptive equipment, splinting / orthotics, wound care, visual-perceptual / sensory-integration, and / or community re-integration techniques.

  • Establishes and revises treatment plan based on physician orders, patient / caregiver input, evaluation results and patient progress.
  • Serves as clinical resource for occupational therapy team.
  • Ensures treatment outcomes meet program standards and reimbursement criteria and facilitate optimal discharge planning.
  • Independently performs all duties required to maintain and complete patient records in timely manner.
  • Communicates effectively with patients, caregivers, other team members to coordinate and educate on the goals and treatment plans.

  • Ensures appropriate delegation to and supervision of Rehab Technician, COTAs, students, interns and volunteers within occupational therapy.
  • Provides occupational therapy team with clinical direction and feedback to optimize patient care by coordinating in-services, meetings, caseload review or co-treatments as appropriate.
  • Assists with communicating and supporting program initiatives.
  • Identifies opportunities for improvement in department programs and / or processes and participates in process improvement projects.
  • Assists with defining and revising occupational therapy policies and procedures.
  • Provides in-services / training in specialty related topics for other hospital or community groups.
  • Ensures safety of patients, caregivers, and co-workers by adherence to health and safety guidelines.
  • Monitors compliance with department cleaning, emergency / safety and infection control plans.

Job Qualifications

  • Graduation from an accredited occupational therapy school with Bachelors or entry-level Master’s degree or verification by the AOTA of equivalent training in another country
  • Current occupational therapy license in State of Illinois
  • Two years of relevant clinical experience required, with a minimum of one year progressive responsibility preferred
  • Specialty certification strongly preferred. Advanced clinical competency in relevant area of practice may be substituted.
  • CPR certification
  • Demonstrated ability to lead and mentor staff
  • Basic English language competence for verbal and written communication in English language
  • Effective interpersonal skills
  • Strong organization and prioritizing skills

    Position Summary :

    Social Workers in hospitals provide frontline services to patients with conditions spanning the entire healthcare continuum.

    Working in concert with the multidisciplinary team, SWs sensitize providers to the social and emotional aspects of a patient’s illness.

    They use their case management skills to help patients and families address the social, financial, and psychological issues related to their overall health.

    The SW assesses the patient’s transitional care needs, performs psycho-social assessments and discharge planning relevant to the patient’s medical treatment.

    Hospital discharge planning is a process that involves determining the appropriate post-hospital discharge destination : identifying what the patient requires and beginning the process of meeting the patient’s identified post-discharge needs (482.

    43 CoP : Discharge Planning).

    Essential Duties and Responsibilities :

    • Performs psychosocial assessments and transitional care screens on all patients as per department and hospital policy.
    • Participates in care planning based on age specific needs.
    • Initiates discharge planning process within first 24 hours of admission or next business day.
    • Monitors patient's length of stay in coordination with the Nurse Case Managers. Addresses barriers to discharge and discusses with the multidisciplinary team.
    • Serves as contact resource fo patients, families and surrogates and meets family and / or significant others to discuss patient’s post-acute treatment needs and discharge planning according to HIPPA and hospital privacy guidelines.
    • Educating patients / families on the levels of health care (i.e. acute, sub-acute, homecare, LTACH, hospice etc.), community resources and goals of care.

    Services may also include non-traditional health care (i.e. transportation, MOWs, homemaker services etc). Provides the patient / family a list of facilities and services in the geographically area requested by the patient.

    • Arranges essential durable medical equipment as ordered by the physician (i.e. bed, wheel chair, infusion, enteral feedings etc).
    • Sends appropriate medical information to providers as per patient preference prior to discharge in accordance with HIPPA and hospital policy.
    • Understands and educates patient / family / staff that patients have the right to participate in the development and implementation of their post-acute discharge care.
    • Coordinates patient / family conferences to discuss treatment plan, goals of care and post-acute needs.
    • Provides consultation to staff regarding psychosocial, bioethical issues and discharge barriers as well as possible community resources.
    • Attends weekly staffing rounds and provides input as appropriate.
    • Assures appropriate communication with referral sources during inpatient treatment, if indicated.
    • Assists with setting up Tele-Psych evaluations as requested by the physicians.
    • Works with psyche intake units to promote a safe transfer to inpatient psyche as per physician referral.
