Reporting to the Transportation Supervisor, the WelbeHealth Driver focuses on transporting participants to and from destinations in a safe manner.
This includes conducting a pre-inspection check on vehicles, and ensuring vehicles are stocked with emergency supplies.
Essential Job Duties :
- Adhere to the assigned schedule of transporting PACE participants, ensuring high customer satisfaction
- Conduct a comprehensive pre-inspection on assigned vehicles, including lift vans, ADA accessible minibuses, and ambulatory vehicles prior to the start of each shift
- Assist clients getting in and out of vehicles, as well as transferring participants in and out of wheelchairs
- Remain alert to pertinent input from other team members, participants, and caregivers and promptly update the Transportation Coordinator or other members of the IDT of any changes in participants’ condition or psychosocial status
- Complete all required documentation in a timely and accurate manner
Job Requirements :
- High School Diploma or equivalency, one (1) year of experience may be substituted in lieu of education
- Current valid State Driver’s License & copy of recent vehicle insurance required
- Must have Driver’s License for at least five (5) years with no more than one (1) moving violation or one (1) at-fault accident in the last three (3) years
- Experience in non-emergency transportation or public transportation preferred
- Must be comfortable using technology

Receptionist
As the initial point of contact for WelbeHealth patients, visitors, and community partners, the Receptionist is accountable for the operation of the front desk.
Acting with the highest standards of professionalism, courtesy, and customer service, the Receptionist welcomes all participants, families, staff, and visitors by telephone and in-person, and facilitates communication between staff, participants, and their families.
Essential Job Duties :
- Greet and interact with patients, families, community partners, and visitors in an inviting and courteous manner, creating a welcoming environment
- Handle the WelbeHealth main phone including :
- Answering the phone promptly and courteously

- Oversee front door, including ensuring visitors sign in and out

Medical Assistant
The WelbeHealth PACE program helps seniors stay in their homes and communities by providing medical care and community-based services.
Our values and participant focus lead the way no matter what. The Medical Assistant will elevate their clinical experience, working closely with providers and our participants.
The Medical Assistant focuses on assisting the medical staff (providers, nurses, and on-site specialty care providers) in the provision of safe and efficient delivery of medical services at the WelbeHealth clinic.
The Medical Assistant also performs routine administrative and clinical duties such as scheduling medical appointments, greeting participants, filing medical records, taking and recording vital signs, preparing exam rooms, and maintaining equipment supplies.
Essential Job Duties :
- Prepare the participants for their appointments, including but not limited to setting up exam rooms, checking participants in and out, obtaining an accurate history of present illness, etc.
- Perform required pre-exam physical tests, like measuring and documenting participants’ vital signs
- Perform general medical procedures, such as dressing changes and suture removal as directed, and under specific authorization and supervision of a physician
- Collect and prepare laboratory specimens as ordered and perform routine sterilization procedures of medical and dental equipment
- Maintain clinic logs and ensure exam rooms are properly stocked, sanitized, and re-prepped for participant encounters at all times
Job Requirements :
- High School Diploma or Equivalency
- Certification by a Medical Assistant Certifying Organization approved by the State Medical Board, preferred
- Two (2) years of Medical Assistant experience in a clinical setting with a frail or elderly population

Social Worker
At WelbeHealth, each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality comprehensive primary care and social services for frail seniors to keep them living safely in the community.
Our values and participant focus lead the way no matter what. The WelbeHealth Social Worker acts as an integral part of the IDT team to help our participants achieve their full potential.
Reporting to the Social Work Manager, the Social Worker maintains a caseload of participants working in close coordination with the IDT.
In addition to delivering standard social work services such as psychosocial assessments, care planning, counseling and case management, the Social Worker functions as a liaison between the participants, family members, support network and care team, as appropriate.
Essential Job Duties :
- Conduct Social Work assessments to determine the psychosocial needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans
- Deliver and document social work interventions as agreed upon in the participants’ care plans including but not limited to arranging necessary resources and services, assisting with care transitions, providing individual as well as group counseling and case management
- Work with the primary care physician and other members of the care team to guide smooth care transitions between settings (e.
g., hospitals, skilled nursing facilities, home, etc.)
- Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and other people within the participants’ support network (family, informal caregivers etc.)
- Provide discharge planning when participants disenroll from the program
Job Requirements :
- Master’s Degree in Social Work (MSW) required
- Minimum of one (1) year of experience with a frail or elderly population
- Previous experience coordinating and facilitating care conference meetings
- Previous experience assisting people with behavioral health & substance abuse issues, preferred

Call Center Representative
The WelbeHealth PACE program helps seniors stay in their homes and communities by providing medical care and community-based services.

The Welbe Advocate II will focus on either inbound and / or outbound assignments within the Call Center (the Hub), which includes but is not limited to call management (answering inbound calls), outbound scheduling, authorization processing, inquiry reply, clinic-based scheduling, and other administrative tasks.
The Welbe Advocate will provide outstanding customer service to all stakeholders (participants, families, partners, and clinic team members), and will be accountable for proper documentation and maintaining current and accurate records of all the scheduled appointments.
Essential Job Duties :
- Answer incoming phone calls, emails, and contacts as requests come in, and appropriately screen, transfer, resolve and dispose of calls expeditiously while adhering to all process and documentation standards
- Review, process, schedule and authorize orders according to department protocols
- Ensure that appropriate and timely documentation of all updates, encounters, and communication is documented in source systems as outlined by department protocols
- Effectively coordinate the scheduling of participants’ appointments, including trouble-shooting conflicts or urgent needs, communicating with all stakeholders (staff, family, providers, etc.

- Provide eligibility status and benefit information for participants
