Manager, Case Management

POSITION SUMMARY / RESPONSIBILITIES

Manages the care coordinators (case managers), home health coordinators and clerical staff to insure a smooth patient flow from preadmission to discharge and follow up in accordance with the patient’s and the payers requirements.

Works with the Manager for Social Services to ensure an interdisciplinary treatment plan of care for patients. Facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the manager of the team.

EDUCATION / EXPERIENCE

Graduation from an accredited college or university with a bachelor's degree in nursing is preferred. Required experience : minimum of two years recent management experience;

three years of hospital clinical nursing experience and one year experience in case management, utilization review or hospital quality assurance.

Experience in each of these areas in a hospital setting is preferred.

LICENSURE / CERTIFICATIONS

Current registration with the Texas State Board of Nurse Examiners and current CPR certification (BLS) is required. Case Manager Certification (CCM or ANCC) is highly desirable.

The Role

  • Cares for comfort and personal needs of patients.
  • a. Provides direct patient care as needed.
  • b. Collects, interprets and documents information related to patient care accurately and efficiently.
  • Performs procedures as directed by the Medical Director. (E, 10%)
  • Maintains quality control logs in area of assignment for CLIA waived labs.
  • Performs CLIA waived labs such as Accu-checks, Urine Beta HCG, Dipstick UA, etc.
  • b. Takes vital signs (temperature, pulse, respirations, and blood pressure).
  • c. Collects blood, urine, stool and sputum specimens
  • Administers medications and immunizations in accordance with clinic policy.
  • Documents and bills appropriately for all nursing charges
  • Provides clerical support to include preparing lab requests, scheduling appointments, contacting patient’s by telephone to reschedule clinic cancelled appointments, obtaining referral authorization for initial visit and any requested follow-up visits to Specialty Care Physician.

Informs patients when referral is authorized and facilitates scheduling of specialty appointments and / or ancillary services and other computer functions as applicable

  • Perform telephone call reminders for scheduled appointments.
  • Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the age-specific and developmental needs of patients served by the department.
  • Appropriately adapts assigned patient assessment, treatment and / or care methods to accommodate the unique physical, psychosocial, cultural, age-specific and other developmental needs of each patient served.

Requirements

High school diploma or GED is required.

Medical Assistant diploma from an accredited school or certification from an approved credentialing body for medical assistants is required.

Must have current American Heart Association Healthcare Provider Basic Cardiac Life Support (BLS) Card.

National certification as a registered Medical Assistant is preferred.

One-year experience in a clinic, doctor's office or ambulatory care setting is preferred

Designated cash handlers must successfully complete the Patient Services Cashier course and be designated as a Patient Services Cashier during the initial probationary period.

POSITION SUMMARY / RESPONSIBILITIES

Provides primary health care and performs selective medical services under the direction of specialty physicians.
