The Case Manager will develop and facilitate the essential case plan components that provide our guests the resources to attain the goal of life transformation and self-sufficiency.

CASE MANAGER ESSENTIAL FUNCTIONS :
- Develop a clearly written case management plan for services and objectives for assigned caseload.
- Perform case management activities to include :
- Intake assessments and evaluations.
- Providing emergency and special service related to legal, medical, or mental health issues.
- Establishing program goals and priorities.
- Conducting scheduled case management meetings with guests.
- Referring residents to essential services such as housing, county benefits, education etc.
- Coordinating plan implementations with other staff.
- Maintain client records and document and submit weekly and monthly-related case management documentation.
- Compose and process various types of correspondence to include letters, sensitive and confidential information, official and / or legal letter and documents.
- Assist with volunteer supervision.
- Travel as required to meet accountabilities.
- Arrange transportation for guests, as needed.
- Conduct other tasks and projects as assigned by the Programs Director.

CASE MANAGER EXPERIENCE, EDUCATION AND LICENSURES :
- Must have High School Diploma or GED.
- Bachelor's degree in behavioral sciences, social work, or a closely related field preferred.
- Minimum 2 years of prior related experience.
- Prior experience working with or serving homeless families is preferred.
- Valid driver's license with the ability to be added to the company's insurance policy.
CASE MANAGER KNOWLEDGE, SKILLS, AND ABILITIES :
- Must have excellent time management skills and ability to multi-task in a high stress environment.
- Ability to communicate effectively in both written and verbal formats.
- Proficient in Microsoft programs including Word, Excel and Outlook and the ability to learn proprietary computer programs.
- Fluency in Spanish, both verbal and written, is a plus.
- Child Development and Special Education background is a plus.

Driver, Class B (Hope Gardens)
Description :
DRIVER, CLASS B POSITION SUMMARY :
The Class B Driver must have passenger endorsement to transport women and children. The Class B Driver will also assist with the management and monitoring of the facility and help with day-to-day tasks which include driving guests and / or staff to appointments or to the bus and train lines;
and help with move outs and events.

DRIVER, CLASS B ESSENTIAL FUNCTIONS :
- Provide shuttle services for employees and residents of Hope Gardens.
- Must be attentive to traffic and weather conditions and ensure passenger safety.
- Enforce a kind conduct and always ensure safety rules on the vehicles.
- Track vehicle upkeep.
- Fuel vehicle(s).
- Maintain records by completing driver logs.
- Maintain a safe vehicle operation.
- Submit random drug tests as required by the California Department of Transportation.
- Actively monitor guests, ensure compliance with all program guidelines and expectations.
- Conduct property / room inspections, as needed.
- Follow established protocols for medical emergencies and unusual / disruptive activity.
- Provide written documentation, as required (may include reports, presentations, and data analysis).
- Facilitate conflict resolution and intervene to diffuse any potential volatile and / or hostile situations, as needed.
- Assist with intake, discharge, and / or orientation processes, as requested.
- Attend scheduled department / team meetings, as requested.
- Conduct tasks and projects assigned by the Program Manager or Executive Director of Program (tasks may include mail sorting and delivery;
- assist in the youth department as needed, transport clients in a company vehicle; serve meals to residents; serve as program representative to volunteers;
and chaperon children at various events, maintain vehicle logs and requests, room inspections, answer incoming phone calls, assist at the front desk).

- Must have a valid Class B California Driver's License with Passenger Endorsement.

- California School Bus Driver Certificate, preferred.

- Responsible for routine cleaning throughout the property, to include mopping, sweeping, vacuuming, dusting, waxing / buffing, carpet cleaning, sanitizing, disinfecting, pressure washing and trash collection.
- Responsible for office and special event room set-ups.
- Assist with moving and relocation of facility furniture and equipment.
- Provides direction to volunteers & apprentices.
- Must be able to travel to other business locations, as needed.
- Must be able to drive a motorized vehicle (Forklift, Car, etc.).
- Conduct other tasks and projects as assigned by the Facilities Manager and / or Director of Facilities.
