Job Summary :

As a Medical Social Worker, you will assess the patient’s psychosocial status, to confirm probably post-acute needs, and to detect any potential barriers to a successful transition in to the community.

You will collaborate with the case manager, the physician and family to determine the best course of action.

You will demonstrate knowledge of human growth and development, assessment, range of treatment appropriate to the ages of patients served.

You will screen all patients on assigned unit / service line by covering the selected number of key screening indicators that consistently signal the need for a more in-depth transition plan evaluation by the social worker.

You will respond to all consults in a timely and efficient manner.

What qualifications you will need :

  • Master's Degree in Social Work required
  • 1-3 years experience in an acute care setting
  • Basic Cardiac Life Support, or CPR must be obtained within 30 days of employment start date

Job Summary and Qualifications

As a Patient Logistics Specialist, you will contribute to the company's mission and vision by providing exceptional customer service to behavioral health community in form of timely responses to phone calls, fax, and e-mails regarding behavioral health inquires.

You will request information regarding admission and transfer.

What you will do in this role :

  • You are responsible for timely response to phone calls demonstrating good customer service skills
  • You will gather information for clinicians to initiate assessments
  • Help locate available resources and coordinate information to support patient disposition
  • You will coordinate with appropriate parties and physicians through the patient response inquiry, admission, treatment and discharge process
  • You are responsible for tracking and reporting on all key indicators related to calls, assessments, and admissions
  • You will gather information and develop reports outlining activities including volume, response time, and outcomes

What qualifications you will need :

  • You must meet basic reading, writing, and comprehension requirements, generally obtained through the completion of high school or GED
  • Bachelors degree in healthcare or related field preferred
  • Prefer 1-3 years' experience in a behavioral healthcare setting
