Overview

The Clinical Data Analyst is responsible for interpreting complex medical information in electronic medical records to support critical program activities including direct patient care and regulatory and accreditation requirements.

This position is responsible for abstraction, analysis, and submission of clinical data to national registries. The analyst applies the policies and procedures of national registries in accordance with Riverside Health System standards and governing national registry criteria to assist in the achievement of institutional goals.

The analyst evaluations data for trends and patterns for apparent cause analysis with the goal to assist Riverside Health System and Riverside Regional Medical Center leadership in making data actionable for ongoing process improvement.

This position is responsible for the quality and coordination of data collection, entry, analysis, submission, report preparation, presentation of data, data sharing for process improvement initiatives, bench marking and outcomes research.

Analyst maintains detailed knowledge of registry field definitions, reporting requirements, has a high degree of organizational skill, and is detailed oriented.

The Clinical Data Analyst is responsible for the technical and methodological support of the assigned registry or registries and reports.

What you will do

  • Conducts and leads process improvement projects based on analysis of clinical data. Collaborates with others in identifying new and improved ways to distribute clear and concise data and display findings electronically to facilitate access to finding, sharing interpretations and expedite decision making.
  • Maintains relationships / mentors the abstractors and their managers. Provides patient lists as required, supports education for the abstractors, coordinates entry, report, analysis, and submissions.
  • Completes special projects as assigned. Runs a variety of reports, including compiling data for analysis, preparing and / or conducting presentations of data analysis and distributing to appropriate team members.

Seeks continual Quality Education for assigned registries to serve as the subject manner expert for the specifications manual.

Recommends appropriate system changes or process improvement plan to improve quality and patient safety initiatives, identifies opportunities to improve patient care experience based on registry data.

Updates team members of data review process changes. Participates in Registry Audits as needed.

Abstracts data for specific cardiovascular populations as needed. Ensures data abstraction is accurate and timely. Proactively and effectively communicates variances and collaborates to resolve issues.

Maintains current reference information materials for registry specific abstraction work

What you will do

  • Responsible for the development and maintenance of routine specialty accrediation and growth metrics. Ensure that monthly comprehensive specialty and retail dashboard data is distributed to key stakeholders.
  • Responsible for the development, updating and maintenance of the accreditation, manufacturers and third party payers data reporting metrics.

Continually work to establish meaningful and useful indicators. Strive to develop indicators that have minimal work involved in updating and maintenance.

  • Works with data aggregators, wholesalers, drug manufacturers and third-party payers to manage and oversee compliance with contractual terms and conditions for data analytics and reporting.
  • Responsible for the development and maintenance of data for specific projects or initiatives as requested by facility or department leadership.

Overview

What you will do

Provides therapeutic services for patients, residents, and their families. Accurately assesses biopsychosocial functioning of assigned patients and residents by using effective interviewing skills, behavioral observation, mental health assessment skills, and collateral sources of information.

Synthesizes information into a written document that provides valuable information for treatment planning. Completes Diagnostic Summaries as assigned.

  • Maximizes use of clinical supervision and apprises Clinical Manager or Program Director of complex or problematic clinical issues.
  • Completes individualized, thorough medical record documentation according to facility and departmental policy. Maintains thorough and timely documentation of case management activities.
  • Employee's individual productivity measures are satisfactory based on FTE / status and average number of assigned patients in case load.
  • Provides quality and clinically sound individual, family, and group therapy interventions based on assessed needs that are appropriate for age, stage of development, and level of functioning.

Participates in the development and revision of the plan of care. Involves the patient / family in the plan of care.

Provides social services to include short-term individual counseling, crisis intervention, assistance in providing information and preparation of advance directives, funeral planning and transfer of responsibilities regarding fiscal, legal, and health care decisions.

Identifies and utilizes appropriate community resources and assesses patient / family ability to access them. Evaluates patient for long-term care when appropriate and assessing ability to accept change in level of care.

Communicates psychosocial information to inpatient facility when level of care changes. Identifies patient / family needs when discharged or when level of care changes.

Evaluates patient / family response to interventions when referred to community agency and satisfaction of the services provided.

Serves as the point of contact for family members and assures they are active participants in the treatment process with patient consent.

Provides education for families on suicidality, diagnosis, and plan of care as appropriate. Addresses patient / family questions and issues.

Assists patient / family in assessing financial resources as appropriate.

  • Supports and benefit patients, residents, department, or facility, in productive ways without compromising patient care duties during non-patient care time.
  • Establishes and maintains solid relationships with referral sources, agency personnel, and community providers. Builds trusting collaborative relationships with an interdisciplinary team to support participant care needs.

Actively participates in interdisciplinary team meetings. Communicates participant and / or caregiver needs to team members, integrates clinical interventions with the plan of care, and evaluates progress on objectives.

Utilizes downtime to maintain entrance points free of debris, discarded cigarettes, trash, etc.

  • Monitors / supports personal and departmental productivity to meet needs of department, i.e., flexing hours, shift changes, time off.
  • Provides appropriate and detailed information in Rcare reports about incidents or concerns with Valet service, damage to vehicles or dissatisfied customers.
  • Demonstrates an understanding of RHS Rcare principles and actively promotes these principles / concepts in daily interaction with guest / patrons using the Valet service.
  • Consistently demonstrates the skills needed to manage time to complete mandatory assignments, suggest ways and opportunities to manage / improve the Valet service, reduce Valet wait times, car retrieval.
