Overview
The Medical Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies.
In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for all patient populations.
This role will receive referrals, and consults for patients from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team) in addition to case finding

- On the basis of referral, consultation or case finding assesses patients and family s psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
- Intervenes with patients and families regarding emotional, social, and financial consequences of illness and / or disability;
accesses and mobilizes family / community resources to meet identified needs : Provides appropriate crisis management to patients and families
- Provides support and counseling to patients / families experiencing and / or anticipating issues to adjusting to illness, catastrophic diagnosis, changes in living situations and bereavement
- Provides intervention in cases involving child abuse / neglect, domestic violence, elderly abuse, institutional abuse and sexual assault.
- Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system.
- Participates in discharge planning activities for patients, in order to ensure a timely discharge and to provide appropriate linkage with post-acute providers.
- Works with families exhibiting complex family dynamics that impact directly on patient care and discharge.
- Works collaborates with the case manager and multidisciplinary team members regarding discharge planning and arranges for the appropriate level of care needed as determined by patients continuing clinical care needs.
- Educates patient / family and physician regarding post-acute options and addresses issues of choice.
- Actively participates and provides pertinent information in complex rounds discharge planning huddles, unit daily whiteboard rounds.
- Functions as the lead in assisting patients and families to obtain available financial and other resources including but not limited to Medicaid, SSDI, County Indigent Funds, UNM Cares, etc.
- Provides intervention in child abuse / neglect, domestic violence, guardianship (temporary / permanent), foster care, adoption, mental health placement, advance directives, adult / elderly abuse, child protection and sexual assault.
- Serves as a lead in obtaining legal guardianship and competency determinations. Completes and disseminates all necessary legal in clinical documentation as required

Provide security for patients, employees, visitors and employees and all those who come in contact with PHS. Have a working knowledge of security equipment and systems as required.

Less than one year experience in general security, however 1 year security experience preferred. Must possess effective communication and written and documentation skills.
Must be able to perform all requirements and training of a security technician. Must be able to demonstrate familiarization of hospital locations and operations within 6 months of hire date.
All facilities excluding PMG sites require the ability to drive a company owned vehicle. These locations must have a current New Mexico drivers license, and must be insurable by Presbyterian Healthcare Services

Responsibilities :
Completes all required training within prescribed time limits.
- Conducts routine and ad hoc patrols of hospital buildings and premises to prevent fire, theft, vandalism, and unauthorized visitors.
- Conducts hazard surveillance for safety, security and infection control issues.
- Completes activity log for each shift worked, detailing activities, accounting for complete shift or duty hours.
- Checks equipment, including patrol vehicle daily, documenting on appropriate checklist.
- Serves as an organizational member of the PHS ADS Hazardous Material / Decontamination team as required.
- Responds to Workplace Violence reports, and notifies appropriate Security Division supervisor as appropriate.
- Conducts investigations, maintains records and prepares incident reports and other documents pertaining to security as required.
- Maintain and enforce established departmental / organizational policies and procedures.
- Prepares written documentation as required by the profession, department and organization
- Interacts and professionally manages issues with employees, physicians, visitors, contractors, and vendors and all who come in contact with PHS.
- Collaborates with local, state and federal agencies as required and approved by PHS.
- Keep supervisor informed of activities and location during the shift.
- Respond to calls for key service, vehicle assist, and escorts in a timely manner.
- Enhances professional growth and development through participation in educational programs, current literature, inservice meeting and scheduled training.
- Secures patient valuables or lost and found articles; maintains logs of items received.
- Controls parking and traffic flow, enforces parking policies and issues citations as required.
- Monitors conduct of visitors on hospital premises; confronts possible unauthorized persons for questioning as needed.
- Gathers necessary information over the radio, phone or in person in a timely, efficient and professional manner.
- Gives appropriate response times to requests for service,
- Reports to work on time, properly uniformed and with all equipment checked and functional.
- Gathers necessary information (pass on) at start of shift to ensure he / she is up to date on current events to adequately perform their duties.
- Maintains a high level of confidentiality as required and reports any confidentiality breaches to proper authorities immediately or as soon as possible.
- Reports all possible complaints to supervisor or director as soon as possible and documents.
- Reports any misconduct of any PHS employee, visitor, physician or anyone who comes into contact with PHS to supervisor, director or appropriate authority within prescribed time limits.
- Maintains all security equipment, uniforms and vehicles as required and reports any issues or misuse to supervisor or director as soon as possible.
- Follows chain of responsibility accordingly.
- Keeps supervisor or director informed of any changes in personal information related to work.
- Attends all mandatory meetings, classes, etc.
- This position will require heavy lifting of greater or equal to 50 pounds for more than 33% of their shifts in situations where self-defense is deemed necessary

Social worker hospice
Overview
This Position is Eligible for a Sign-On Bonus of up to $21,000!
The Outpatient Hospice Social Worker is responsible for the development, planning, implementation and evaluation of psycho / social related interventions for patients followed in the Home and Transition services programs.
This position will be responsible for identifying patients who have complex psychosocial needs, require assistance with eligibility determination for social programs funding sources and who may qualify for community assistance from a variety of special funds and agencies.
In addition, the Hospice / Palliative Social Worker operates as the mental health professional of the team. All interventions are aimed to improve Quality of Care / Life and decrease complications and readmissions.
The Outpatient Hospice Social Worker, offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of the Plan / Goals of care for all patient populations served by HATs teams.
This role will receive referrals, and consults for patients from at risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team) in addition to case finding.

Responsibilities
- Provides Hospice / Palliative / Care social work across outpatient and inpatient settings.
- On the basis of referral, consultation or case finding assesses patients and family s psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
Completes comprehensive psychosocial assessment. Provides psychosocial interventions.
- Manages with the IDT the patient plan of care. Provides strengths-based, person and family centered care.
- Intervenes with patients and families regarding emotional, social, and financial consequences of illness and / or disability; Provides psychoeducation.
- Provides supportive counseling to patients / families experiencing and / or anticipating issues to adjusting to illness, catastrophic diagnosis, changes in living situations and bereavement.
- Intervenes with patients demonstrating complicated grief and explores bereavement related to possible death and multiple aspects of loss and adaption along the continuum of care.
Provide anticipatory guidance.
- Provides non-pharmalogical pain and symptom management, reinforces patient and family personhood and coping as they negotiate complications with chronic illness and end of life.
- Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system.
- Facilitates family meetings. Participates in discharge planning activities in collaboration with other hospital staff for patients, in order to ensure a timely discharge and to provide appropriate linkage with post-acute providers.
- Works with families exhibiting complex family dynamics that impact directly on patient care and discharge.
- Provides clinical guidance and consultation to interdisciplinary primary teams, including social workers and case managers, prompting continuity of care as well as safe and comprehensive care coordination and discharge planning as indicated,
- Educates patient / family and physician regarding post-acute options and addresses issues of choice.
- Provides intervention in child abuse / neglect, domestic violence, guardianship (temporary / permanent), foster care, adoption, mental health placement, advance directives, adult / elderly abuse, child protection and sexual assault as appropriate
- Participates in on call rotation.
