This position will also have light maintenance duties to support the daily operations of Maintenance.

4. Responsible for maintaining schedule to include last minute delivery / pickup requests.

7. Fuel vehicle and submit receipts into Admin

This position is responsible for :

Facilitating the adjustment of the patient and family to illness and to help the family and the individual function optimally.

The social worker will provide psychotherapy and psychosocial support for children with a broad array of diagnoses, including but not limited to adjustment to medical illness, ADHD, depression, anxiety and autism spectrum disorders.

Services are provided to assist the family in the best utilization of the health care system and link them with the variety of services necessary to meet their individual needs.

The social worker must be knowledgeable of age specific principle of growth and development. The social worker must possess effective communication skill needed to collaborate with all members of the health care team.

1. Manage complex assessments of the patient / family's biopsychosocial situation and establishes goals.

2. Periodically reassesses the patient / family situation and modify plan as needed.

3. Provide services to address social needs of patients receiving care in their assigned clinic, including but not limited to, domestic violence, psychiatric emergency cases, child abuse, family disputes, end of life and bereavement, Social Determinants of Health, and compliance with treatment.

4. Participate in the interdisciplinary assessment and treatment of patients and their families, and provide consultation for physicians and other members of the health care team to determine current psychosocial needs.

5. Work with the multidisciplinary team to address the mental and behavioral health needs of the patients.

6. Act as liaison between hospital and the various community agencies, which provide aid to families and children.

7. Coordinate and provide therapy sessions for patients showing a need for counseling / therapy. Provide counseling and / or therapy for patients and their families assisting them with emotional, personal, psychological, socioeconomic factors, and adjustment to illness issues, to include the entire spectrum from diagnosis throughout the treatment course.

8. Document interventions in the Electronic Medical Record following departmental guidelines.

9. Follow Medicaid, CMS and other regulatory / compliance agency requirements for documentation and billing.

10. Interface with insurance providers, case managers and others involved in the authorization of needed patient services.

11. Establish and maintain effective working relationships across disciplines.

Qualifications-

Masters degree in Social Work from a program accredited by the Council on Social Work Education (CSWE) and active LCSW licensure required.

The ideal candidate will manage and provide care for the patient requiring neonatal intensive care utilizing collaborative consultation with team members, under the direction and supervision (but not necessarily the direct attendance) of an attending neonatologist.

Additional responsibilities include the following :

  • Reviews and provides general care items involved in neonatal care, such as temperature stabilization, hyperalimentation therapy, feeding management, and phototherapy.

Job Description
