JOB SUMMARY
Reporting to the Manager of Therapy Services. Develops and implements individual physical therapy programs designed to improve and maximize patient mobility and independent functioning within their physical limitations / disabilities.
Provides treatment on an inpatient or outpatient basis.
JOB QUALIFICATIONS
- Current state of Illinois licensure as a physical therapist.
- A Bachelor’s Degree or certificate in Physical Therapy from an American Physical Therapy Association approved school which would provide an understanding of anatomy, physiology, pathology and physics necessary to assess patient condition and develop treatment plans and goals.
- The interpersonal skills necessary to communicate with patients and all levels of hospital personnel.
- The analytical ability required to evaluate a patient’s progress and response to treatment and to plan and adjust treatment plans.
- The ability to effectively and accurately operate all department equipment related to physical therapy treatments.
JOB DUTIES
- Evaluates the patient’s functional abilities / disabilities including range of motion and muscle testing. Through assessment and testing procedures establishes a patient’s functional baseline measurement to be used throughout treatment as a gauge of progress and physical improvement.
- In accordance with patient evaluations, establishes treatment plans which specify objective treatment goals and the treatment modalities that will assist the patient to regain function, increase strength and decrease pain.
Modifies treatment plans as the patient’s condition changes.
Performs physical, electrical, mechanical and heat treatments including but not limited to facilitation, strengthening and mobilization exercises, gait training, prosthetic training, massage, cervical and pelvic traction, ultrasound, hot pack application, Jobst RX, transcutaneous electrical neural stimulation, whirlpool;
pulsed lavage, iontophoresis; biofeedback, trains patient with DL / adaptive equipment and in an appropriate home exercise program.
Supervises treatments provided by physical therapist assistants. Reviews treatment plans and patient progress with fellow treatment team members.
Instructs and supervises ancillary personnel in the proper technique / treatment.
As necessary discusses treatment plans and patient progress with the physician. Consults with social services / case management, home health agencies, and the patients’ families regarding discharge planning;
Including but not limited to : the regimen of home treatment, the patient’s physical level of independence, and level of nursing care needed.
- Instructs and trains the patient / family or other responsible person in home exercises, transfers and ADLS as appropriate.
- Documents the patient’s treatment and progress in patient’s chart (or outpatient records) indicating the condition in relation to their established medical goals.
Completes Medicare records and progress notes. Co-signs physical therapist assistants’ documentation.
- Schedules patients, arranges transport service while noting special equipment or handling where indicated. Indicates patient treatments on billing tickets.
- Provides in-services concerning physical therapy to hospital staff and / or the community in physical therapy.
- Keeps abreast of developments in the physical therapy field and incorporates the approved developments in treatment and the evaluation plan.
- Assists in Quality Improvement monitoring and analysis.
- Meets department productivity measures.
- Conforms to the established Standards of Behavior.
- Other duties as assigned.

JOB SUMMARY
Participates in the preparation and service of all hospital feeding programs, assuring quality and sanitation consistent with department standards.
JOB QUALIFICATIONS
- High school diploma or GED or five years comparable experience
- Requires the ability to read, write, speak, understand and follow written or verbal instruction in English
JOB DUTIES
- Demonstrates the WMH Customer Service Values, which are key in providing quality service to patients and customers.
- Stocks workstations with necessary items to be ready by service times.*
- Serves food to customers / patients in a timely, appealing, and friendly manner.
- Ensures portions and service methods meet department guidelines.*
- Monitors food quality at all times for appropriate temperature, freshness and customer acceptability.*
- Controls waste through minimizing leftovers, portion control and proper stock rotation.*
- Minimizes expenses by properly handling of China and Serviceware.
- Utilizes a team approach in carrying out daily activities. Demonstrates and maintains a professional attitude with all customer / patient interactions.
- Attends 100% of the department / hospital in-services annually.
- Always wears hairnets and gloves where appropriate and keeps hands clean at all times. Sanitizes workstation after cleaning.
Follows all Board of Health and JACHO standards. Labels and date all food items.
- Cross-trained in other Dietary positions and fills-in as needed.

Social Worker

JOB SUMMARY
Provides social services appropriate to patients’ needs to address psychosocial adjustment, discharge planning, access of appropriate resources.
Responsible for decision making in accordance with clinical, fiscal, and organizational objectives. Functions as liaison between patient and family and the case management team.
JOB QUALIFICATIONS
1. Graduation from an accredited school of Social Work with Master’s degree
2. Current social work license in State of Illinois preferred or obtained within first year of hire
3. CPR certification
4. Basic English language competence for verbal and written communication in English language.
5. Knowledge and skills necessary to provide age specific care, appropriate for the types of patients served.
6. Effective interpersonal skills to establish therapeutic relationships with patients / caregivers, interdisciplinary team members and external contacts.

- Develops, coordinates and implements discharge plans in collaboration with patient and family.*
- Schedules patient / family education sessions with the rehabilitation team.*
- Monitors progress of patient towards expected outcomes in relation to goals by conducting team conferences, facilitating weekly rounds and gathering data from pertinent sources.*
- Records in Midas necessary information to keep health care team apprised of goals, progress and outcomes.*
- Coordinates communications both internally within the rehab team and externally with other involved hospital / medical professionals.*
- Assists with managing care and resources according to clinical and fiscal objectives and regulatory requirements.*
- Communicates with Weiss’ case managers in confirmation of insurance certification and authorization of services with third-party payers, external case managers or other insurance liaisons to effectively manage use of resources.*
WORKING CONDITIONS
Physical Environment / Hazards : Infrequent exposure to diagnosed and undiagnosed infectious and communicable diseases. There is potential of exposure to inadequate ventilation and air conditioning and frequent exposure to multiple interruptions.
OSHA Category II : Normal routine involves no exposure to blood, body fluid or tissue, but exposure or potential for exposure may occur.
TB Exposure : There may be an occupational risk for exposure to TB and the incumbent is required to take TB control training, PPD skin tests and / or health screenings.
