Case Manager Position Summary
Under the direct supervision of the Texas Clinical Director, the part-time case manager is primarily responsible for the provision of case management and intervention services to youth and families identified as at risk by the school.
Responsibilities include : advocacy; referral and linkages; Student Support Team development; Student Support Team interventions to increase the youth’s school success.
Case Manager Responsibilities :
Implements and coordinates case management services to youth and families designed to reduce truancy, decrease problematic school behaviors;
improve school success, and prevent the entrance into or further penetration into the juvenile justice system
- Conducts intakes and assessments
- Documents appropriately
- Develops student support teams in conjunction with the youth and family
- Develops, coordinates, and implements individual, family, group, and team interventions
- Provides intervention and support to families in crisis intended to keep the family unit intact
- Collaborates with school personnel and student supports
- Attends and participates in individual supervisory conferences, staff meetings, training sessions and professional development opportunities as required.
Maintains required program certifications.
Case Manager Qualifications
- Bachelor’s degree in social work or related behavioral health field with experience providing services to youth and families.
- Excellent verbal and written communication skills.
- A commitment to cultural humility to support students, staff, and co-workers
- Strong organizational and interpersonal skills. Effective oral and written communication skills. Ability to work independently and with flexibility in days and hours of work.
Demonstrate initiative, sound judgment, and an understanding of issues affecting youth and families identified as at risk by the school.
- Must have a valid State of Texas driver’s license and access to reliable transportation.
- Must be able to meet directly with families and stakeholders in the community.
