SOCIAL SERVICES CASE MANAGER
PART-TIME SOCIAL SERVICES CASE MANAGER - JEFFERSON COUNTY, CO Family Resource Specialist SALARY RANGE : $20.00 - $24.00 PER HOUR We are seeking an experienced and compassionate Social Services Based Case Manager to join our team.
In this role, you will support families in our community by providing them with resources, advice, and assistance. You will be responsible for connecting families with local services, such as housing assistance, healthcare, and other support services.
As a Social Services Case Manager, you will also provide guidance and direction to parents and families in need, helping them find the resources they need to create a better future for their children.

- Master’s or Bachelor’s Degree. Preferred areas of study include social work, criminal justice, psychology, or related field and a minimum of one (1) year direct care experience or supervised practice in lieu of the experience requirement.
- Valid driver’s license, auto insurance and access to a vehicle.
- Pass current, applicable clearances.
- Pass pre-employment, post-offer drug screening.
- Ability to work non-traditional hours including evenings and weekends.
The qualified Social Services Case Manager will :
- Assess and engage families in service and treatment planning based on demonstrated needs.
- Plan and implement appropriate interventions using evidence based and best practice approaches.
- Assist families in accessing and navigating community resources.
- Attend court hearings and provide testimony as required.
- Transport clients utilizing personal vehicle, as necessary.
