Position Summary : The Occupational Therapist (OT) is a Member of the Interdisciplinary Team of the San Diego PACE Program and is responsible for the design and implementation of a program of occupational therapy for PACE patients to help them to remain independent and live safely in the community.
The OT performs assessments of PACE patients and develops individualized plans of care, is responsible for determining the appropriate approach to be used in developing function to a maximum level and supervises the carrying out of the plan of care by support staff.
Performs reassessments as indicated.
Essential Functions of the Job :
- Develop and implement a program of occupational therapy for patients enrolled in PACE and accessing day services.
- Perform assessments of patients to determine abilities.
- Develop and implement an individualized plan of care to decrease or eliminate disability, increase or maintain capability for independence, and develop function to a maximum level.

- Guide patients and caregivers in the use of adaptive devices and therapeutic, creative and self-care activities for improving function.
- Perform assessments as indicated. Evaluate the effectiveness of treatment and modify goals as necessary. Traveling required as indicated.
- Conducts home, hospital, and other site visits, assessments and interventions as needed and indicated by IDT and / or Center Manager.
- Record and sign treatments in the patient's health record. Record, sign, and date quarterly progress notes. Review and initial all evaluations and the discharge summary.
- Ensure services provided are in a therapeutic setting conducive to restoring dignity and self-esteem to all patients.
- Ensure that required equipment and supplies for the occupational therapy program are available or ordered as needed.
- Develop personnel policies which define the occupational therapy responsibilities and the duties assigned to the occupational therapy assistant.
- Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government / regulatory agencies (PACE, CMS, DHCS, etc.)
- Participate in training programs for staff and caregivers.

- Maintain and follow established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
- Enhance professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
Education Required :
Graduate of an OT curriculum accredited by the Committee on Allied Health Education and the Accreditation of the American Medical Association and the American OT Association.
- Licensed Occupational Therapist in CA
- Passing scores on national and state examinations.
- CA valid driver’s license and reliable transportation
- Proof of current vehicle insurance (if applicable)
Experience Recommended :

Schedule : Monday - Friday from 9 : 00 to 6 : 00

The Passenger Driver provides door-to-door transportation to all Program of All-Inclusive Care for the Elderly (PACE) participants and makes any other deliveries as needed in a positive and helpful manner.
In addition, the Driver cleans, conducts scheduled vehicle safety inspections, performs routine maintenance, cleaning, and paperwork.
Responsibilities :
- Safely operates company vehicles to transport participants to and from their homes, sites, off-site medical appointments, and / or activity outings.
- The Driver provides physical assistance and monitoring to participants from the pickup or drop-off location to the vehicle and as they get on and off the van, lifting as necessary.
May occasionally be instructed to go into participants’ homes.
- The Driver reports any special situation in participants’ homes, or while on the road, to the Road Supervisor including changes in participants’ behavior or appearance that may indicate an alteration in medical condition.
- The Driver provides other transportation as directed, including medication or supply delivery.
- Performs regular maintenance of the vehicle : Cleans daily; completes daily maintenance check list; ensures that vehicle operates effectively with good brakes, lights, and other equipment necessary for safe operation;
checks gas, oil, and tires daily.
- Reports all unusual incidents or accidents and completes appropriate reports.
- Accurately completes, and turns in, daily route sheets and other paperwork in a timely manner.
- Maintains courteous, respectful attitude towards participants and their families.
- Maintains cooperative, professional work relationship with other members of treatment team.
- The Driver observes all traffic laws and is conscientious and respectful to pedestrians and other drivers.
- Participates in the quality assurance / performance improvement program (QAPI) as assigned.

Job Overview
The Primary Care Physician oversees the medical care of senior participants in a state-of-the-art outpatient clinic setting, with some local travel to patient homes or skilled nursing facilities.
The PCP will usually see 10-12 patients per day, with adequate time provided for charting, patient follow-up, and care coordination.
