Licensed Clinical Social Worker - LCSW - Behaviorist
Harney District Hospital of Burns, OR is looking to hire a full-time LCSW to work with HDH primary care providers to assist patients with behavioral needs and support.
This position starts at $34.46 and increases based on experience. We also offer exceptional full-time benefits, including $1,000 / month towards medical & vision, $65 / month for dental, choice of retirement plans, and up to 200 hours of paid time off (PTO) per year.

You will provide an active presence to the HDH Family Care clinical teams and be available to accept warm-hand-off's and provide assessment and brief treatment interventions to patients referred by the medical providers.
You will f acilitate referrals and coordination to other services (e.g. substance abuse treatment, tele-psychiatry, or community resources) as needed or as directed by primary care provider.
You will d evelop and maintain professional relationships and act as a liaison with the local mental health agency to enhance coordination of patients being referred for services at their facility.
Your work will help patients effectively resolve individual barriers through follow-up, advocacy and collaboration with staff and other community service providers.
MANDATORY REQUIREMENTS : Master's degree in social work from accredited school.
