RN Case Manager
Description
Good Shepherd Hospice is seeking a compassionate candidate for a Register Nurse Case Manager serving patients and their families in Clear Lake, TX Southeast, and surrounding communities.
The RN provides nursing assessment, planning and care to maximize the comfort and health of patients and families. The RN responds to all phone calls promptly and appropriately, makes patient visits as necessary and completes after hours admissions and deaths as needed.
- 8am to 5pm, Monday thru Friday
- Previous experience in hospice, home health, or geriatrics preferred

- Responding to Phone Calls Promptly
- Professional Management of the Patient and Family
- Communicating with Medical Professionals, the Hospice Team and Others
- Training the Patient and Caregiver
- Elevating Necessary Matters to the Attention of the Patient Care Manager
- Cultivating Communication and Bereavement Skills
- Maintaining Accurate, Up-to-Date Records
Skills Knowledge and Expertise
- Hospice Experience preferred
- Current Registered Nurse License
- Strong Organizational and Self-Management Skills
- Critical Thinker with Good Judgment
- Must be of high integrity including maintenance of confidential information including all patient records
- Works as a team player, following directions and adhering to defined care plans
- Demonstrated commitment to the hospice philosophy of care
- Can successfully pass background checks
- Valid Driver's License
