POSITION SUMMARY :
Be the first smiling face for patients, family members and visitors be our Receptionist!
DUTIES & RESPONSIBILITIES :
- Answer and direct incoming calls
- Take / deliver messages to personnel
- Answer questions for callers regarding the organization including address, directions, etc.
- Welcome on-site visitors, assist them appropriately, and always provide excellent customer service to both the visitors and fellow staff members.
- Perform other administrative or clerical duties such as filing, photocopying, and collating as needed.
QUALIFICATIONS & REQUIREMENTS :
- Must have a minimum High School diploma or equivalent G.E.D
- Must have 1-3 years of relevant experience, preferably in a healthcare environment
- Must have excellent phone etiquette, communication skills, and customer service skills
- Must have working knowledge of computers, internet access, and the ability to navigate within Microsoft Office Suite, or similar programs
- Ideal candidate is outgoing, upbeat, organized, and demonstrates excellent customer services skills
Licensed master social
DUTIES &RESPONSIBILITIES :
- Assist in the planning, developing, organizing,implementing, evaluating, and directing of the Social Services Departmentwithin the facility in accordance with current existing Federal, State andlocal standards
- Develop and manage the initial contact asnecessary with the discharge planner, family or responsible party to initiatetour of facility and follow up procedures
- Assist in developing and implementing policiesand procedures for identifying the medically-related social and emotional needsof the patient / resident
- Develop and maintain a good working rapport withintra-department personnel, other departments within the facility and outsidecommunity health, welfare and social agencies to ensure that social serviceprograms can be properly maintained to meet the needs of the patients / residents
- Keep abreast of current federal and stateregulations as well as professional standards
QUALIFICATIONS &REQUIREMENTS :
- Must have a current Social Work license, in goodstanding with the state (as required)
- Master’s Degree in Social Work required
- Must have at least 3+ years of experience in along term care or behavioral health facility
- Ideal candidate must be a skilled communicator,director and motivator
Case Manager LPN
POSITION SUMMARY :
Case management is a collaborative and truly holistic process. Case Managers work diligently alongside doctors, nurses, social workers, and many other medical and non-medical personnel to meet the complex needs of patients.
They ensure that the care and discharge plans meet the physical, social, and emotional needs of the patient.
DUTIES & RESPONSIBILITIES :
- Effectively manage benefit, utilization of services and resources
- Review to determine appropriateness and medical necessity, continued stay and the use of ancillary services using concurrent review process
- Work effectively developing, enhancing and maintaining relationships with third party payers
- Function as a liaison between facility and managed care / insurance company
QUALIFICATIONS & REQUIREMENTS :
- Must have at least 2+ years of Case Management Experience
- LPN license in good standing with the state (as required) preferred
- Must have a Bachelor’s degree (in a health-related field is preferred)
- Case Management Certification preferred
Marketing Liaison
POSITION SUMMARY :
This position is all about relationships and connections. Developing and maintaining relationships with people, both internal and external customers.

DUTIES & RESPONSIBILITIES :
- Maintain effective markets for facility services by developing and sustaining referral relationships
- Coordinate external marketing of facilities within the community through health providers and organizations
- Develop strategies to maximize admissions of residents in accordance with facility and regional sales plan
- Evaluate effectiveness and cost benefit of marketing tactics
- Make recommendations to alter strategies, and develop marketing tools for facility or district within their budget constraints
- Participate in weekly sales team meetings, reporting regularly on referral development and progress on targets and reviews census management practice guidelines, analyzes trends, and acts to avert deterioration of market share
QUALIFICATIONS & REQUIREMENTS :
- Must have at least 1-3 years of experience in health care, long-term care industry, or insurance sales / marketing & 1+ years of experience with the proposal development process
- A bachelor’s degree in marketing or a related field is preferred
- Public relations experience is preferred
- Must have strong technical writing skills and be familiar with medical terminology
- Must have superior presentation skills (both 1-on-1 and in a group setting), and must be proficient in Microsoft Office products (Word, Excel, PowerPoint)
