Assistant Medical Director - Physician
Job Description
Primary Accountability
The Assistant Medical Director is responsible to provide and oversee the provision and supervision of professional care services of Doctoral level or Mid-Level Providers in a Patient Centered Health Home context, with a focus on access to care.
Description of Primary Responsibilities
1) Provides management to departmental staff.
a) Responsible for performance management of assigned staff, including recognition, performance evaluations, and formal coaching and counseling.
b) Demonstrates core leadership behaviors and team one approach.
c) Creates a culture of accountability and excellence
d) Develops and manages an action plan across assigned employee base to support the strategic direction and obtainment of goals of the organization, effectively leading change when necessary.
e) Empowers staff through effective communication and talent building.
f) Delegates appropriate departmental duties and responsibilities to assigned team members while ensuring department performance.
g) Assists with the development of the departmental budget and monitors budget to ensure expenses do not exceed budget.
h) Ensures regulatory compliance for assigned departments, and compliance with all workflows, policies, and procedures.
i) Ensures employees receive instruction / training that is in compliance with training plan, including on the job training.
Works with supervisor to ensure necessary remediation is taken.
j) Recommends workflow, policy, and procedure changes based on observations from performance metrics, outcomes, and feedback from supervisor.
k) Ensures department maintains compliance with all employee related reporting and tracking.
2) Responsible for providing direct provision and supervision of professional care services of Doctoral level or Mid-Level Providers.
a) Implements clinical protocol and guidelines.
b) Establishes goals.
c) Addresses site provider needs assessments on an on-going basis.
d) Assists in professional staff recruitment activities.
e) Prepares the orientation schedule for Doctoral level or mid-level staff members.
f) Ensures room assignments and scheduling for professional staff.
3) Responsible for assisting in the design, implementation, and execution of programs at the Network.
4) Responsible for the direct provision of primary care medical services meeting all established productivity expectations.
a) Will provide examination and will review appropriate laboratory, referral and imaging results to determine a diagnosis.
b) Expected to document visits accurately utilizing appropriate templates, and record relevant data to attain a diagnosis.
c) Accountable for attaining budgeted visits.
d) Accountable for meeting clinical care measures.
e) Accountable for locking records in the 72 hour window.
5) Responsible for meeting Midlevel professional focus and goals.
a) Provide comprehensive, continuous and coordinated medical care for acute and chronic illness at all ages within the context of family unit in a community setting.
b) Comprehensive health care including health promotion and maintenance, prevention of illness and disability, and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
c) Management of chronic diseases and acute illness with broad range knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
d) Advocate access to health care with focus of structured evaluation, early intervention and health promotion.
e) Consults with supervising physician or other team members on cases that need further input.
f) Works within and helps to promote the Patient Centered Health Home model of care.
6) Accountable for providing excellent customer service and care.
a) Consistently behaves with courtesy when interacting with patients / family members, support staff, and Network staff.
b) Provider remains aware of wait time during the daily workflow.
c) Responsible for providing the patient with educational materials and resources appropriate to the patient’s health literacy needs.
7) Accountable for patient status as measured by relevant clinical quality measures.
a) Promotes and participates in daily huddles.
b) Incorporates the participation of ancillary health team members in managing the care of patients / families.
c) Responsible for setting and monitoring self- management plans.
d) Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
e) Completes chart and peer review responsibility in a timely manner.
8) Assists the patient care team with performance improvement efforts, staff education and FHCN promotion.
a) Responsible for assisting the patient care team with performance evaluations and quality improvement.
b) Where relevant participate in rotation of medical, physician assistant students and dental residents.
c) Participates in Network Health Fairs & other community events.
d) Participates in site & provider meetings in a meaningful way.
9) Meets Network and credentialing expectations for licensure, and employment.
10) Performs other duties as assigned.
Description of Primary Attributes
Professional & Technical Knowledge :
1) Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer and technical skills, and mathematical knowledge frequently acquired through completion of a Bachelor’s Degree program with a recognized major or comparable experience, and;
2) Four years of leadership experience or 5 years of progressively greater responsibility or significant contributions to projects and initiatives that demonstrate leadership skills.
3) Performance of the duties and responsibilities of the job requires extensive formal training in Medical Studies, including an understanding of the application of the theory and practices of Medicine learned during completion of an accredited medical school program.
Technical Skills :
1) Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs and other elements.
2) Ability to use Microsoft Excel to analyze data, including the use of formulas, functions, lookup tables and other standard spreadsheet elements.
3) Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
Licenses & Certifications : Licensed to practice medicine in the state of California.
Communications Skills :
1) Job duties require the employee to effectively communicate, verbally and in writing, their opinions and extrapolations of information they collect and synthesize / analyze.
2) Responsible for the resolution of conflicts that may arise because of disagreements between employees, between employees and customers / clients, or with the public, other legal entities or governmental authorities.

Medical Assistant - Back Office
Job Description
Description of Primary Responsibilities
1) Responsible for supporting and monitoring patient flow in the back office.
a) Scrubs appointments in advance to ensure necessary information is available for patient visit.
b) Assists in the distribution of patients, including walk-ins.
c) Keeps patients informed of wait times and ensures patients are properly flowed in system.
d) Schedules patient appointments.
e) Follows-up on no-show appointments and ensures messages and requests are responded to accordingly.
f) Understands general front office workflows and is able to step in to support the front office when needed.
2) Responsible for performing various aspects of patient visit.
a) Reviews health record with the patient and updates clinical information such as current medication, medical, and surgical histories.
b) Obtains vital signs and patient data.
c) Initiates the follow-up of specialty care referrals and services per policy.
d) Supports dispensary related functions, including requests for prescription refills, logging of medications, and pharmacy / patient phone calls.
e) Completes necessary clinical forms required for patient visit such as patient assistance programs, PM-’s, and Staying Healthy Assessment forms.
f) Maintains continuous communication within the patient care team.
3) Responsible for performing important medical tests and procedures on patients.
a) Prepares necessary instruments, room set up, and obtains consent for required procedures.
b) Performs Quality Control of CLIA waived screening equipment.
c) Performs CLIA waived screenings and properly documentation in system.
d) Performs injection techniques.
e) Preforms standing orders and clinical indicators such as immunizations, foot screening, and Tuberculosis screening.
f) Performs specimen collection, preparation, and labeling.
g) Performs veni-puncture procedures (if Venipuncture certified).
4) Responsible for obtaining medical record releases, and ensuring the processing of medical record requests.
a) Scans relevant documents into the electronic health record system.
b) Performs documentation in the patient’s electronic health record according to policy and training.
5) Responsible for preparing and stocking exam rooms / lab and monitoring adequate supplies in stock.
6) Responsible for discarding of Biohazard waste and sharps.
7) Responsible for weekly testing and weekly / monthly cleaning of sterilization equipment and the sterilization of medical equipment.
8) May also be responsible for maintenance of the mobile health unit
a) In charge of weekly cleaning and restocking of mobile health unit, dumping septic tank and refueling the vehicle.
b) Drives mobile health unit to and from various designated locations (as often as multiple times per day, after-hours, and on weekends)
c) Maintains all vehicle logs and any DMV records as required.
d) Assists disabled passengers and children into and out of vehicle.
e) Immediately reports to supervisor any vehicle safety, maintenance and hazard issues. Cooperate with any 3rd party official or employee assigned to vehicle inspections.
9) Performs other duties as assigned.

2) Injection certification is required. As an alternative, a venipuncture, injection and skin test certificate can be completed as part of the initial job specific competencies.
3) Valid California driver’s license required.
