Under the direction of the Manager of Call Center, the Patient Service Rep-Call Center is responsible for performing customer service functions, from responding to a high volume of inbound calls, scheduling appointments, pre-registering , handling issues / complaints, responding to questions, or directing patients to the appropriate area.
This role may also assists the Manager of Call Center with training new staff and mentoring peers.
Job Responsibilities
- Answers telephone and assists patients by providing information or directing them to the appropriate area, in a pleasant and professional manner.
- Assists patients in scheduling, rescheduling, or canceling appointments and documenting the reason for the visit.
- Enters patient data into computer system accurately and schedules appointments correctly by following the provided appointment guidelines.
- Performs other duties as assigned.
- Reports any unusual circumstances or phone call issues to the Manager of Call Center while following procedures outlined in training guides, memos, and other FHCSD documents.
- Responsible for attending and putting into practice all staff training activities related to appointment scheduling.
Requirements
- At least 1 year of experience in a call center preferred.
- At least 2 years customer service experience required OR Medical Assistant Certification / Medical Terminology course required.
- High School diploma or GED equivalent preferred.
- Medical Assistant Certification or Medical Terminology course preferred.
Experience / Specialized skills (including Language)
- Ability to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high-workload environment.
- Bilingual English / Spanish required.
- Intermediate computer literacy (i.e., use system to manage and schedule appointments, access electronic medical record information).

Prenatal Case Manager
Responsibilities :
Assists in patient flow. Ensures that test results and referrals are given to appropriate team members and accurately recorded.
Reviews medical records prior to appointments to ensure completion and accuracy.
Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
Ensures that test results, referrals, and reports are given to appropriate team members for review and are accurately recorded.
Reviews medical records prior to appointments to ensure completion and accuracy. Scans various medical reports into electronic health records following established protocols to ensure information is available through hospital portal at time of delivery.
Coordinates client's individual care plans and oversees completion of all recommendations made on the care plan. Assists clients with practical arrangements such as translation needs, referrals, special appointments, Medi-cal eligibility, and utilization of internal and external services (e.
g., HIV testing, pediatrics, speech & hearing).
- Ensures cases are closed following established standardized protocols.
- Keeps prenatal logs (pregnancy testing, master list, abnormal pap, etc.) current and provides appropriate and timely follow-up.
- Performs other duties as assigned.
- Provides individual client orientation, education, combined assessments, and reassessments as needed. Works closely with team, Lead, and / or Supervisor to meet department productivity goals.
- Provides pregnancy testing and counseling to clients in an unbiased manner, providing appropriate referrals.
Requirements :
- 1 year experience in perinatal or women's health care preferred.
- Bachelor's degree in Health Science or Social Science field preferred.
- High School diploma or GED equivalent required.
- Medical Assistant diploma or LVN desirable.
- Ability to accurately and quickly perform data entry.
- Ability to work effectively with patients from diverse social, cultural and economic groups.
- Ability to work well in both a team-based environment and independently.
- Bilingual in Spanish / English required.
- Demonstrated ability to be culturally sensitive and respect diversity.
- Good written and verbal communication skills.
- Knowledge of perinatal programs, care management, and procedures.

Healthcare Enrollment Specialist
Responsibilities :
- Conducts health insurance options education at appropriate agency and community sites. Attends community meetings and trainings as needed.
- Documents contacts, maintains files, and submits program statistics as needed. Utilizes an enrollment verification system to document outcomes.
Timely and complete submission of project logs / data reports.
- Meets department application assistance productivity standards of 4 applications per day.
- Performs other duties as assigned.

Requirements :
- Certified Enrollment Counselor Certification must be obtained within 90 days of hire and maintained while in this position.

- Attends in-services or completes make-up assignments.
- Keeps sites in compliance with TJC, DHS, CHDP, and Medi-Cal Managed Care Requirements.
- Performs Waive Testing and Phlebotomy.

Requirements :
- Medical Assistant diploma from an approved course is required.

Responsibilities :
- Ability to accurately complete data entry at relatively high speed, utilizing grammar and spell check software.

Social Worker, BSW
Responsibilities :
- Conducts thorough psychosocial assessments to determine when clients need psychosocial interventions; provides periodic psychosocial reassessments for clients.
- Maintains a list of current community emergency resources (food, clothing, shelter, counseling) for each assigned site / program.
Partners with community resources and assists clients with external resources as needed.
- Provides psychosocial interventions. Facilitate individual, family, and group counseling and therapy.
- Provides referrals as appropriate. Promotes internal referrals to FHCSD services (e.g., Pediatrics, HIV testing, CFIS, FCC, etc.).
- Works as a member of the multi-disciplinary healthcare team, providing input on patient cases and working cooperatively with the healthcare team to create proactive solutions and comprehensive treatment plans.
