DUTIES :

  • Effectively screens / evaluates patients with physical functioning disorders and / or wound care and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders and / or wound care per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to : manual techniques; ambulation;

therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.

  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.

Qualifications :

Licensed Physical Therapist

Duties :

  • Effectively screens / evaluates patients with cognitive and / or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and / or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to : ADL retraining; therapeutic activities;

therapeutic exercises; modalities; use of supportive and assistive devices; etc.

  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.

  • Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to / from dining room, etc.).
  • Serve food trays. Assist with feeding as indicated (i.e., cutting foods, feeding, assist in dining room supervision, etc.).
  • Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood / body fluids.
  • Inform the nurse Supervisor / Charge Nurse of any changes in the resident’s condition so that appropriate information can be entered on the resident’s care plan.

Your duties in the position will be to provide primary care to specific residents under the medical direction and supervision of the resident’s attending physicians or the medical director of the facility with an emphasis on assessment, illness prevention and health care management.

  • Confer with the Medical Director and the attending physician regarding specific residents assigned to you.
  • Consult with the physician concerning resident evaluation and assist the Director of Nursing Services in planning and developing the nursing services to be performed for the resident.
  • Determine when to refer the resident to a physician for evaluation, supervision, or directions.
  • Perform administrative duties regarding assigned residents, such as completing medical forms, reports, evaluations, studies, charting, etc.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Supervises and assists in management of the infection control program including ensuring that personal protective equipment is used in the handling of infectious materials by nursing personnel.
  • Observes medication passes and treatments to ensure quality.
  • Administer services within the applicable scope of nursing practice, which may include : catheterization, tube feedings, suction, applying and changing dressings / bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead / dying, etc.

  • Prepare and administer medications as ordered by the physician.
