Empath LIFE, a member of Empath Health, is currently seeking a Physical Therapist to join our team.
The Physical Therapist :
- Under the supervision of the Empath LIFE Medical Director, provides physical therapy services to participants of Empath LIFE
- Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, ALF, Nursing Home or in-home setting.
- Integrates the physical therapy treatment plan into the Interdisciplinary Team comprehensive plan of care.
- Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.
Position Requirements
Be a graduate of a physical therapy curriculum approved by one of the following : The American Physical Therapy Association;
The committee on Allied Health Education and Accreditation of the American Medical Association; The council on Medical Education of the American Medical Association and the American Physical therapy Association required.
Other organizations approved by the Secretary.
- Licensure :
- Current State of Florida Physical Therapy license
- Valid Florida State Driver's License
- Certifications :
- Valid CPR certification from the American Red Cross or American Heart Association.
- For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion.
Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
