Custodian
Summary :
Cleans and maintains company facilities, including washrooms, meeting rooms, offices, carpet and flooring, furniture dusting and restocking restrooms daily which will provide a safe and healthy workplace.
Essential Duties and Responsibilities :
Core duties and responsibilities include the following. Other duties may be assigned.
Works as part of a team to carry out cleaning tasks to ensure standards are maintained.
- Empties trash receptacles and bags trash for proper disposal, identifies appropriate cleaning materials and products to sanitize area(s), disinfect and deodorize according to company standards.
- Replenishes supplies in washrooms, such as hand soap, paper towels and toilet paper.
- Organize cleaning items, disinfectants and other materials kept in storage, as well as cleaning equipment.
- Uses proper signage to indicate wet flooring or other potentially hazardous situations.
- May oversee and / or assist with snow removal activities.
- Changing of light bulbs as needed.
- Lock and unlock assigned buildings : secure building when facilities are not in use checking for unlocked doors and windows, report any unauthorized occupants, turn off lights.
- Follow instructions regarding the use of chemicals and supplies. Use as directed.
- Maintain accurate logs and records of areas that have been cleaned and schedule for regular cleaning.
- Fulfils work order request in case of cleaning or plumbing emergencies.
- Ensures any and all repairs noticed during cleaning duties are reported to supervisor in a timely manner.
- Ensures cleaning task assigned by supervisor are carried out in a timely fashion.
- Liaises with team members, supervisor and organization leadership.
- Follow cleaning routine and schedule as set by supervisor including sweeping, mopping, vacuuming, window washing, mirrors, etc.
- Attend safety meetings and other related meetings.
- Assist with setup / breakdown of facilities for meetings, classrooms, conferences, events etc.
- Assist with inventory control and security.
Education and / or Experience :
High School / GED
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year to year basis.
Attention to detail.
Good record keeping skills, outstanding communication skills, and excellent interpersonal skills.
Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be drug free.

- Using power equipment to buff and restore floors and shampoo carpets

Case Manager
SUMMARY
This position is primarily responsible for managing program services for enrolled participants including the development of care plans, provision of linkages and referrals to care and follow-up activities, communication with medical / behavioral health providers and project staff to decrease barriers to access for medical care and support services.
Assists in outreach and health education activities, as needed. Recruits individuals for, and conducts, evidence-based prevention interventions and counseling.
Conducts data entry. This position reports to the Program Coordinator.
Essential Duties and Responsibilities :
- Conduct GPRA Intake Assessments and Follow-up Assessments;
- Utilize SBIRT model, including PHQ-9, CAGE, PCL-C, LEC, PSQ, and GAD-7 assessment tools;
- Manage written and electronic participant records;
- Participate in agency-sponsored and community outreach events, such as health fairs, LGBT events, etc.;
- Provide presentations for internal and external programs to promote the program services;
- Educate participants on substance use, HIV, and HCV prevention issues;
- Recruit for, implement, and facilitate Seeking Safety EBP;
- Assist with development and ongoing project implementation plan;
- Participate in bi-weekly supervisory sessions with the Program Coordinator;
- Complete documentation regarding participant assessments, consent forms, and demographic and contact information;
- Conduct ongoing outreach & retention efforts via one-on-one contact and program events;
- Re-engage participants for Follow-up Assessments at 6-months post-Intake Assessment;
- Participate in b-weekly staff meetings, case conferences, and in-house and external trainings;
- Ability to travel off-site to conduct outreach and attend necessary trainings and conferences;
- Perform other duties as assigned.
Qualifications :

Education and / or Experience :
Bachelor’s Degree and CASAC required; at least 3 years of experience in HIV / AIDs or behavioral health; Certified in HIV / Viral Hepatitis Testing and Pre / Post Testing Counseling required;
bilingual in English / Spanish preferred.
- High School Diploma or GED CASAC-T with 4 years’ experience working with applicable populations who have substance abuse / co-occurring disorders.
- Good verbal and written communications skills
- Knowledge of HIV / AIDS, substance use & mental illness, and LGBT issues
- Good problem-solving skills and ability to work autonomously
- Must have computer skills (Microsoft Office, etc.)
- Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be drug free.
Language (English / Spanish) a plus.
