Duties & Responsibilities :

  • Greet visitors and employees at the front office
  • Answer Phones and route appropriately

We are looking for a Care Services Coordinator (CSC) to work with our Ensemble Care Management division to enhance the quality of client management, maximize satisfaction and promote cost effectiveness.

The successful candidate will be accountable for the full client care cycle. CSC duties include reviewing client status, planning, implementing, monitoring and analyzing actions required to meet the client’s health and human services needs.

The CSC will also work to facilitate, coordinate and assist with the provision of essential services to help patients transition through a variety of settings including hospitals, transitional care facilities, skilled nursing facilities, and private homes to ensure continuity of care across locations.

Work will be completed both in an office environment as well as community settings. Bilingual English / Spanish is a plus.

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handle case assignments, draft service plans, review case progress and determine case closure
  • Help clients achieve wellness and autonomy through education and engagement
  • Assist clients in utilizing technology designed to improve health outcomes including remote patient monitoring equipment
  • Facilitate multiple care aspects (case coordination, information sharing, etc.)
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop effective working relations and cooperate with medical team throughout the care coordination process
  • Coordination of patient resources with social services, health and governmental agencies
  • Go the extra mile and interact with clients to keep track of their progress and to ensure satisfaction
  • Record case information, accurately complete all necessary forms and produce statistical reports as needed
  • Promote quality and cost-effective interventions and outcomes
  • Address motivational and psychosocial issues as needed
  • Adhere to professional standards as outlined by protocols, rules and regulation
  • Respond to customer queries in a timely and accurate way, via phone, text, email or chat
  • Collaborate / communicate with medical and non-medical staff, including Payer clinical management teams

Requirements

  • Experience in case management or coordination strongly preferred
  • Understanding of case management principles, healthcare management and reimbursement
  • Skilled in clinical practice and interpersonal relationships; able to cope with family emotional stress and tolerant of individual lifestyles
  • Able to make meaningful observations and write comprehensive, accurate reports in a timely manner
  • Training and / or experience in researching medical records, using internet search engines, and have a working knowledge of spreadsheets and word documents
  • Demonstrate ability to assess and respond to the needs of patients / families in varied settings
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • Bachelor's or Master's degree in social work
  • State driver’s license and availability of car
