Position Summary

The Social Worker is a professional who provides services to residents at the facility and their families or other responsible parties in accordance with current Federal and State regulations and facility policy and procedure to assure that the psycho-social needs of the residents are met and maintained on an individual basis.

Qualifications :

  • Bachelor’s Degree in Social Work or Human Relations field including but not limited to sociology, special education, rehabilitation, counseling, or psychology and one year of supervised social work experience in a health care setting.
  • Ability to communicate effectively and tactfully with residents, families, visitors, administration, staff, other service providers, and the community.
  • Ability to work harmoniously with others, both individually and as a team member.
  • Physical capabilities and emotional stability to function adequately for the welfare of the residents.
  • Ability to function independently and flexibly as required to meet resident needs in both routine and crisis situations.
  • Ability to complete documentation in accordance with government regulations and facility policy.
  • Personal integrity and willingness to work within the ethical principles of the facility and the social work profession.

Essential Duties and Responsibilities

Facilitates and processes admission of new residents to the facility with the Admission Coordinator. Completes all new admit social service paperwork including, but not limited to social history, PASRR, MDS, discharge plan, and care plan.

Care Planning Participate in care conferences, complete, update, and implement care plans with team goals.Discharge Planning Initiate discharge plans.

Provide information and assistance to families and residents. Coordinate follow up services.Direct Service to Resident Provide individual and group counseling as appropriate.

Ensure resident rights are maintained. Provide information and assistance as needed.Family Support Involve families with resident care, care planning, and other possible needs related to discharge planning, regulations, and community services.

Provide brief counseling if needed.Advocacy Develop rapport with staff, other service providers, and families. Provide staff training on resident rights, abuse and neglect, psychosocial needs, and confidentiality.

Documentation Completed assessments, notes, care plans, and other documentation in a timely fashion, accurately, and with Periodic Reviews.

Departmental Duties Participate in in-services and meetings as well as keeping abreast of regulation changes related to social service and make appropriate changes.

Position Summary

Essential Duties and Responsibilities

Facilitates new resident admissions by obtaining a social history, completing required paperwork with residents / responsible parties (EX : PASSR), distributing paperwork to appropriate staff, completing an assessment of the resident’s cognitive, emotional, psychosocial and behavioral status

Provides information and support to residents and families at admission, throughout the resident’s stay, and at discharge

Resolves problems and complaints

Serves as an advocate for resident rights through advising residents of their rights, consulting with staff and others about resident rights, and by providing staff training on resident rights and psychosocial needs

Counsels privately with residents as needed and provides individualized interventions to assure the resident’s cognitive, emotional, social and behavioral needs are being met (EX : reality orientation, validation, reassurance, behavioral management, empowerment, advocacy, TLC, grief counseling, etc.)

Assesses residents’ current condition and needs through visits with residents, contact with responsible parties, consultation with staff, record review, and observation

Participates in regularly scheduled Care Planning Conferences with the Interdisciplinary Team, completes required documentation, and facilitates implementation of team goals

Initiates discharge plans for residents within fourteen days of admission, makes revisions as needed, provides a full update of discharge plans annually, and assists residents who are planning discharge by providing information and coordinating follow-up services

Provides family support by involving families in the care planning process, providing information on resident care, condition, relevant regulations, financial and legal issues, referral to community resources, and through counseling

Completes and maintains required forms / reports

Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures.

Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor / manager or to the Corporate Compliance Officer.

The Physical Therapist is a licensed professional (LPT) responsible for evaluating, planning and implementing physical therapy service to patients in their home or current residence as prescribed by the attending physician.

The therapist works as a member of an interdisciplinary rehabilitation team.

QUALIFICATIONS

  • Graduate of a state approved school of physical therapy accredited by the American Physical Therapy Association
  • Minimum of 6 months acute care or rehabilitation center experience. Outpatient therapy experience may be considered if practice included medical / surgical patients
  • Licensed as a Physical Therapist in the Commonwealth of Pennsylvania
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a car with insurance limits as established in personnel policies

Clinical Skills

  • Performs the initial start of care assessment for patients admitted for therapy only within 24 hours of receiving the referral
  • Develops the plan of care cooperatively with the physician, patient / family and other team members with revisions based on the patient’s condition
  • Instructs patients, families and other health care team members in the total physical therapy program including, but not limited to the home exercise program, the proper and safe use of prosthetic, orthotic and assistive devices and safe transfer activities
  • Supervises the assigned home health aide every 14 days when the physical therapist is the coordinating health professional
  • The admission, carried out after adequate orientation, is comprehensive in nature and follows assessment guidelines
  • Plans with the patient, family and other team members for the patient’s discharge from service

Physical Therapy Assistant (PTA) SUPERVISION

The Licensed Physical Therapist is responsible for the supervision of the care provided to specific patients by the PTA.

The PTA may not carry out the initial Physical Therapy evaluation, develop the care plan, secure physician’s orders, or independently discharge the patient.

Supervision is carried out by :

  • The Licensed Physical Therapist will provide an on-site visit and actively participate in the patient’s treatment once every 7th visit or every 14 calendar days, whichever occurs first, to re-evaluate the patient based on treatment response, clinical needs, functional status and goal progression.
  • The LPT is responsible for the PTA competency evaluation, any necessary remediation, and the performance evaluation of the PTA.
  • All notes written by the physical therapy assistant will be co-signed by the supervising licensed physical therapist.

Documentation

  • Completes the PT evaluation within a reasonable time frame per the patients clinical status after acceptance of the referral to determine the immediate care and support needs of the client, otherwise specified by the physician or requested by the clients with permission of the patients physician.
  • Completes required documentation for on-going services on the day of the visit
  • Completes the discharge order and summary / transfer
  • Prepares patient record for recertification, as appropriate

Communication

  • Documentation should include : physician name, date, time, discipline, frequency and duration of visits and signature of PT
  • Reports to the primary nurse / therapist a summary of the patient’s condition including;
  • Problems concerning the patient and how these may be resolved
  • Changes in patient needs and how these can be met
  • The need for involvement of other members of the health team such as nursing, home health aide, speech / language pathologist, physical therapist and social services
  • Patient progress towards goals and functional status
  • Reports patient responses to treatment, changes in care needs, progress towards goals to the supervising Director of Outpatient Rehab
