What The Case Manager Does :
- Understand and remain up-to-date on all CAMBA programs and services that may be beneficial to NYPH patients.
- Research external community programs and services that are complimentary to CAMBA programs and develop relationships with community partners.
- Respond by phone to NYPH patient referrals within 48 hours.
- Educate eligible individuals about service offerings and health benefits of the program.
- Support completion of intake paperwork and comprehensive assessments with patients who consent to the program. This may be by phone, video, email, mail, or in-person.
- Provide short-term case management services for approximately 1-2 months, with at least weekly follow-up.
- Enter client data into CAMBA database within 24 hours (i.e. contact information, assessments, client services, progress, outcomes, etc.)
- Work with clients to break through barriers to their goals and to assist them in advocating for themselves and in moving toward self-sufficiency.
- Assist clients in completing applications for benefits and entitlements (e.g. Medicaid, etc), and process applications on clients’ behalf.
- Monitor clients’ progress towards their service goals, and document all interactions with clients via progress notes.
- Create and maintain client files and make copies of all client documents.
- Collaborate with other CAMBA programs, community partners and NYPH team members (such as : patient navigators, community navigators, social workers, care managers, and care coordinators) to improve patient outcomes.
- Participate in outreach, planning and execution of special events, including workshops, health fairs, and jointly agreed upon community events.
- May follow-up with clients for a period of time after successful completion of their primary goals to assure client stability.
MINIMUM EDUCATION / EXPERIENCE REQUIRED :
Bachelor’s Degree (BA or BS) and two years of applicable experience, and / or equivalent experience.
OTHER REQUIREMENTS :
- At least 2 years case management or direct client care experience, required.
- Bi-lingual English / Spanish and / or English / Creole speakers strongly preferred.
- Knowledge about, understanding of, and ability to work closely with, persons with chronic illness and a variety of needs related to the Social Determinants of Health.
- Knowledge of community resources and sensitivity toward persons with chronic illnesses and multiple social needs.
- Follow all HIPAA privacy rules as they relate to personal health information and documentation.
- Ability to utilize public transportation to navigate the five boroughs of New York City.
- Ability to utilize mobile technology as a tablet to document while performing field work.
Compensation : $45,000-$50,000 annually
Status : Full-Time (35 hours per week), some flexibility with hybrid schedule of office and remote work.
