Discharge Planner
JOB SUMMARY
Coordinates the discharge planning with the patient attending physician and will assist patient and family in contacting appropriate community resources.
Makes patient & family aware of Medicare / insurance benefits and qualifying factors and the patients personal discharge goal will always be considered with input from the family.
ESSENTIAL FUNCTIONS :
- To promote discharge planning services to facilitate a timely discharge to a safe and healthful environment.
- To ensure the right to each patient, to reach her / his highest level of independent wellness through the support of post hospital and community services as needed.
- To carry out discharge planning interventions as ordered by the physicians, implementing them with the individual needs of the patient as a priority, while respecting the patient’s personal beliefs and values.
Local agencies will be consulted and community resources utilized whenever necessary.
- To maintain competence of discharge planning staff and evaluate based on performance standards in the job description.
- To provide orientation for any new employee and continuing education of discharge planning staff.
- To interpret, implement, and uphold facility policy.
- To ensure coordination among all departments within the hospital in furthering the mission of the facility.
- To foster and maintain positive public relations
- To maintain required reports, records, and statistical information.
- Provides assessment of each patient meeting established criteria.
- Focuses the plan of care and services on the patient / family needs, preferences and advocacy using multidisciplinary approach.
- Provides facility and community resource information for patient and families to make realistic choices about post-hospital care plan and to facilitate the implementation of the established plan.
- Provides a continual system of communication through documentation and re-evaluation of the patient considering the anticipated needs at the present level of health care delivery and thereafter.
- Provides a consistent method of patient care management, which maximizes the hospital length of stay thereby decreasing the risk of readmission’s related to noncompliance and inappropriate utilization of resource.

Responsibilities
- Implement standards of care for medical social work services
- Participate in patients’ plans of care.
- Perform patient evaluations and help develop a treatment plan with patients’ physicians
- Assess the psychosocial status of the patients as related to their illness
- Make follow-up visits to assess and continue the plans of care
- Plan interventions based on patients’ needs and findings
- Maintain accurate and up-to-date records
Qualifications
- Master’s degree in social work
- Minimum one year of experience in health care and social work
- Current CPR certification
- Valid driver’s license
- Psych experience is a plus
