Position Summary :

The Alignment Jump Start Assessment program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients with complex and chronic care needs.

As a JSA Advanced Practice Clinician, you will work within an interdisciplinary team environment that includes physicians, nurse practitioners, physician assistants, case managers, and other health care team members to ensure proper delivery of clinical and home-based patient care.

In this role, you will assess, develop, and coordinate options and services promoting quality care in order to achieve optimal health care outcomes while also ensuring cost-effective care complying with Alignment policy and all state and federal regulations.

Our members are typically aged 65 and over and home and virtual visits will be between 45 and 60 minutes depending on whether it is an annual or initial visit.

General Duties / Responsibilities :

May include but are not limited to)

JSA APC’s responsibilities

Conduct initial or annual face-to-face patient visits in home and clinic settings

Follow CMS guidelines to make virtual visits when appropriate)

  • Health Risk Assessment includes : Past medical history Review of symptoms Physical examination Medication review Cognitive / depression screenings Fall assessment / functional assessment Immunizations and preventive health screenings including Colonoscopy, Breast Cancer screening, Bone density, and diabetes eye exams Advance Care Planning and POLST
  • Update and reevaluate the year-to-year diagnoses following CMS guidelines and HCC coding
  • Identify diagnoses and develops proper care plan for chronic disease management
  • Provide patient / family / caretaker education and facilitate patient empowerment and quality of life
  • Matching care desired with care given while providing true, informed consent
  • Collaborate with HEDIS / STARS and other Alignment programs and departments to facilitate optimal program integration and continuity of care
  • Refer patient to an appropriate program such as Care Anywhere Program
  • Refer patient to social worker for appropriate community resources as needed
  • Provide contact information for patient to receive medical assistance to avoid unnecessary emergency room visit and hospitalization
  • Comply with all HIPAA regulations and maintains security of protected health information (PHI)
  • Complete EMR documentation with quality that meets Alignment expectations in a timely manner.

  • Computer literacy / EMR proficiency
  • At least one (1) year prior clinical experience

Education / Licensure :

  • Active California state license as a Nurse Practitioner / Physician Assistant

Summary :

In this role you will assess and evaluate members’ needs and requirements to achieve and / or maintain their health. You will be providing field, virtual, and telephone visits.

You will guide members and their families toward and facilitate interaction with resources appropriate for their care and well-being.

You will also work in collaboration with a multi-disciplinary team, employing a variety of strategies, and techniques to enable a member to manage their physical, environmental and psycho-social health issues.

General Duties / Responsibilities :

  • Conduct telephonic Social Work outreach to assigned members to assess health, environment, mental health, nutrition, functionality, decline, and psycho-social areas of concerns by conducting a Social Work assessment.
  • In response to assessments, coach and problem solve with member to identify and address specific goal(s) to support health and behavior change.
  • Timely documents Social Work Interventions and goals in the EMR. Adheres to the departments documentation requirement of submitting face to face field visit documentation within 2 business days of seeing the Member.

Documents all telephonic and virtual visits into the medical record the same day care is provided.

  • Promotes the value of Health Care Advanced Directive and documents discussion of member preferences.
  • Charts thoroughly and timely on all Member encounters.
  • Provide appropriate interventions to optimize health and well-being. Interventions may include education, the coordination of community- based support services, supportive counseling for mental health conditions, care navigation, advanced care and end of life planning
  • Collaborate with other members of the interdisciplinary team.
  • Charts member's treatments and progress in accordance with state regulations and department procedures.
  • Makes referrals to case manager, as appropriate, and / or refers member's family to community support services and resources.
  • Provide home assessment to high-risk members and develop an individual care plan
  • To better serve members and implement the model of care, will understand the clinical program design, program monitoring and reporting.
  • Perform a full range of clinical Social Worker procedures in accordance with clinical privileges granted by the Plan and based in accordance with Social Work standards of practice.
  • Practice as an interdependent member of the health team and provide important components of primary health care through direct Social Work Services, consultation, collaboration, referral, teaching, and advocacy.
  • Provide direct and indirect services to both inpatients and outpatients in accordance with Social Work standards of practice.
  • Assess and treat outpatients in individual and family modalities exercising mature professional judgment and using a wide range of social work skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute diseases / injuries.
  • Conduct psychosocial assessments to determine patient needs and resources (both family support and community support).

Provide counseling to patient and family in matters directly related to patient’s limitation, adjustment to medical condition, and ongoing treatment.

Participates with Nurses and Physicians in the implementation of discharge plans, follow-up care, and transfers to other health care facilities (e.

g., nursing homes, rehabilitation hospitals, etc.)

  • Plan and maintain referral and coordination services of services with other agencies to provide optimal patient care.
  • Provide consultation services to medical, nursing, and ancillary hospital staff regarding psychosocial issues, discharge plans, and follow-up care for patients and families.
  • Provide crisis intervention services when indicated.
  • Respond independently, and with various media, to appropriate community requests. Take the initiative in seeking out opportunities to present programs to meet the needs of patients / members and their families.
  • Consults with hospitals and plan in the coordination of care regarding the mental health of members. Develop and maintain working relationships with community resources.

Coordinate with physicians, and representatives of their service disciplines for the benefit of the member and their families.

Take initiative in identifying and assessing the needs of the community and organize responses to address those needs.

  • Act as a human services agent, using clinical judgment and knowledge of area resources to provide information and referrals to patients and other care providers.
  • Interface with the RN Case Manager(s) and the Interdisciplinary Team (IDT) in the development and implementation of Social Work interventions.
  • Integrate Social Work case management and nurse case management as a team.

PLEASE NOTE THIS IS A TEMPORARY POSITION

Medical Assistant is responsible for completing all of medical assistant duties with a high demonstrated competency in addition to managing patient flow on the floor.

Additional duties such as special projects, reports, administrative or compliance tasks may be assigned as needed.

General Duties / Responsibilities :

May Include but are not limited to)

  • Assist with Drop-in home visits as requested by the assigned provider (Driving up to 100% of the time)
  • Complete all basic patient intake with demonstrated competency including but not limited to : Vitals (height, weight, blood pressure, pulse, respiration, temps, etc) Phlebotomy Vaccine Administration Medical History / medication collection including Jump Start Application and EMR Other machines / systems trained on by supervisor and within Medical Assistant Scope
  • Complete all patient check in functions including eligibility verification, appointment scheduling, confirmation, arrival of member within the EMR system and checkout.
  • Complete HEDIS and all other clinical alerts tasked to a medical assistant role per protocols including management of home monitoring systems if applicable
  • Deliver on excellent customer service for every member including assisting with care coordination needs such referrals, enrollments in chronic disease management programs, etc.
  • Facilitate member concerns, complaints or coordination needs regarding services at Alignment clinics, primary physician’s offices, any network provider or with health plan.
  • Complete telephonic history and pre-visit planning as needed
  • Complete patient drop in visits for those that are unable to contact or need equipment assistance in home for Alignment’s telehealth programs
  • Facilitate referrals / authorizations for members between the member, Alignment, specialist and primary care provider if applicable.

Minimum Requirements :

Minimum Experience : 3 years' experience as a medical assistant preferably in a physician office including working knowledge of managed care.

Experience working with internal medicine offices highly preferred. Knowledge in Medicare and Medicaid benefits and resources preferred Familiar with medical terminology

Education / Licensure : Requires a high school diploma. Medical Assistant Certification is preferred. LVN / LPN or other higher nursing certifications accepted as well.

If MA Certificate not available, dependent on the state scope of practice regulations, minimum 3 years Medical Assistant experience demonstrating medical back office competencies.

Current Driver’s License CPR / First Aid Certification is required.
