PRN Position

Requirements

  • MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
  • One year of experience in a Hospice Care preferred
  • Valid Driver's License, car insurance and reliable transportation

Job Duties :

  • Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
  • Provides care, under the direction of a physician and participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
  • Documents all patient / family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record.
  • Conducts reassessments of patient / family needs and counseling as required.
  • Provides crisis intervention and individual or family counseling when indicated.
  • Plays an active role as a care advocate for the patient / family unit.
  • Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
  • Provides ongoing counseling related to issues of death and dying to the patient and family as needed.
  • Participates as a member of the Bereavement Team as assigned.
  • Adheres to all Hospice policies.

Compensation : Approximately $97,479 - $131,004 per year. All individual pay rates are calculated based on the candidate's experience and internal equity.

Overview of Role :

High proficiency in Spanish required.*

Provide high quality medical care that is culturally appropriate and prevention focused. Serves as a leader on the Pod contributing to the smooth, efficient, operation of the Pod, the Site and the Organization.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Provides patient care as per Health Care Plans and within his / her Scope of Practice.
  • Provides high-quality, comprehensive care to all patient populations, both in acute and chronic contexts (e.g., applies planned care principles;

practice variations from Clinica’s treatment protocols as appropriate; limitations of training / experience are respected).

Expands and maintains rapport with the medical community in the service area (e.g., interacts appropriately with other medical personnel outside of Clinica;

actively seeks to broaden our referral network).

  • Appropriately communicates with health & human services agencies, local governments, and community organizations as needed.
  • Provides responsive off-hours medical services physically or by phone when the clinic is closed, if applicable.
  • Supports and teaches team members at the site. Teaches students enrolled in a relevant health-profession training program.
  • Demonstrates knowledge of the principles of growth and development over the life span, and stays current with changes in this area.
  • Provides Human Resources staff the needed documentation, paperwork, and signatures to complete credentialing and privileging (initial and biannual).
  • Is respectful and responsive to staff and patients (is approachable by other staff, demonstrates good team work by offering and receiving feedback appropriately).
  • Handles ongoing daily clinical work in a timely and thorough manner. This includes being present for huddle, responding to messages (by phone and e-mail), staying on time while caring for patients, charting, managing the PAQ and Tasks, submitting referrals, reviewing records, and completing paperwork needed for optimal patient care.
  • Exhibits positive leadership skills on and off the pod (actively striving to improve the patient and staff experience of Clinica).
  • Collaborates well with teammates, both on a clinical level (e.g., seeks / provides guidance and feedback to supervised / supervising clinician) as well as on a managerial and administrative level (e.

g., follows Clinica’s procedures around requesting / taking time off).

  • Handles ongoing daily clinical work in a timely and thorough manner as outlined in the Provider manual (see Provider Manual).
  • Effective use of the EMR, registries and templates.
  • Works under the supervision of a designated physician.
  • Supports and promotes MCC utilization with the Clinician team.

POSITION QUALIFICATIONS :

Education and Experience :

  • Bachelor’s Degree.
  • Graduate of an accredited Nurse Practitioner or Physician Assistant Program.
  • Licensed in the State of Colorado.
  • Eligible for DEA licensure (new graduates should obtain within a year of hire).

Knowledge, Skills and Abilities :

  • One year of experience in ambulatory health care preferred.
  • Sensitivity to low income, ethnic minority communities preferred.
  • Proficiency in the Spanish language.
  • Computer proficiency EMR experience preferred.
  • Ability to flourish in a team system.

    Overview of Role :

    Full-Time Schedule : Monday-Friday from 7 : 45am-5pm. 40 hours per week.*

    This is an in person position at our Pecos clinic.

    Ensure incoming calls are handled professionally, courteously and efficiently and callers have their needs met and a satisfying experience when calling Clinica Family Health.

    ESSENTIAL DUTIES AND RESPONSIBILITIES :

    • Answer incoming calls professionally.
    • Responsible for intake of New Patient that includes creating charts, scheduling and educating New Patients.
    • Verify and update patient demographics with each patient related call as needed.
    • Screen callers to determine needs.
    • Screen for duplicate charts and follow duplicate chart process.
    • Screen for bad address alerts and follow bad address process.
    • Follow Alerts on patients’ chart to schedule and transfer appropriately.
    • Schedule, cancel, confirm or reschedule appointments according to the scheduling guidelines.
    • Transfer calls and take / send messages according to protocol.
    • Answer caller questions according to protocol.
    • Provide exceptional customer service to all callers.
    • Maintain cultural sensitivity when dealing with customers and coworkers.
    • Take part in collaborative working relationships in the organization that include participation in PDSA’s, work groups or committees.

    Report back relevant information to the CC Manager and Supervisors.

    Responsible for creating Patient Experience Forms on behalf of caller’s request and sending them to the appropriate departments.

    Should we create these or send them to the Ops phone at the pertaining site?

    • Responsible for updating preferences to ensure access to our patients.
    • Responsible for reading, responding to and managing communications via Outlook regarding organization and departmental updates.

    POSITION QUALIFICATIONS :

    Education and Experience :

    • High School diploma or GED preferred.
    • One year phone service preferred.
    • One year medical facility preferred.
    • Customer service experience required.
    • Strong telephone etiquette and excellent keyboard skills.
    • Excellent ability to quickly grasp complex guidelines, protocols and rules for scheduling and managing patients.

    Knowledge, Skills and Abilities :

    • Fluency in Spanish preferred but not required.
    • Good telephone etiquette and excellent keyboard skills.
    • Excellent grasp of Call Center guidelines, protocols and rules for scheduling and managing patients.
    • Excellent interpersonal skills enabling the person to be able to react effectively and calmly in emergencies and within strict guidelines.

    • Good attention to detail.
