Responsibilities
- Evaluate patient’s functional status and home environment
- Identify equipment needs and participate in the development of the total plan of care
- Develop treatment program and establish goals for improved function
- Teach new skills, retrain in once familiar daily activity skills, fabricate splints, train in use of prosthetic and / or orthotic devices
- Maintain appropriate clinical records and communication with other personnel involved in patient’s care
- Supervise Occupational Therapy Assistants according to organization policy and state regulations
- Participate in discharge planning
- Participate in case conference as required
- Ensure compliance with all state and federal legal and regulatory requirements
- Additional responsibilities as assigned
Qualifications
- Graduate of an occupational therapist education program accredited by ACOTE with current licensure to practice in state of operation
- 2+ years occupational therapy experience
- Home health experience preferred
- Commitment to clinical and documentation excellence
Social Worker - PRN

Responsibilities
- Assessing the psychosocial status of patients and families / caregivers related to the patient’s illness and the environment and communicates findings to the registered nurse and other members of the interdisciplinary group
- Carrying out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse, and plans interventions based on evaluation findings
- Counseling patients and family / caregivers as needed in relationship to stress and other identified coping difficulties, providing crisis intervention when necessary
- Assessing for any special needs related to the culture of the patient and family
- Maintaining clinical records on all patients referred to social work
- Participating in the development of the individualized plan of care involving the patient and family, and attending regularly scheduled interdisciplinary group meetings
- Assisting physicians and other team members in understanding significant social and emotional factors related to health problems and death / dying issues
- Assisting family and patient in planning for funeral arrangements, financial, legal and healthcare decision responsibilities
- Ensuring compliance with all state and federal legal and regulatory requirements
- Additional responsibilities as assigned
Qualifications
- Graduate of a Master’s program in social work accredited by the Council on Social Work Education
- Current licensure according to Medicare guidelines to practice in state of operation
- 1+ year experience in healthcare social work
- Experience in home health or hospice preferred
- Excellent verbal and written communication and organization skills
- Commitment to clinical and documentation excellence
