Case Manager

Join the team of highly dedicated mental health professionals. We are presently looking for a Case Manager who will be responsible for assisting the treatment team by facilitating the treatment process, by reviewing and being familiar with initial team assessments and treatment plans, along with being familiar with patient goals and needs.

The Case Manager documents patient goals and progress toward goals, utilizing recommendations and consultations of the treatment team.

The Case Manager will make necessary contacts to carry out the discharge plan.

ESSENTIAL FUNCTIONS :

  • Provide case management and care coordination services for clients / patients / residents in the facility.
  • Responsible for coordinating continuum of care activities for assigned clients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.
  • Conduct various duties related to coordinating treatment and discharge activities, as needed.
  • Provide quality care through developing, implementing, managing and evaluating client / patient care plans.
  • Collect, correlate and provide clinical data to the treatment team.
  • Coordinate in a timely manner, issues or activities relevant to the treatment team.

EDUCATION / EXPERIENCE / SKILL REQUIREMENTS :

Bachelor’s degree in Social Work, Counseling or related human service field required.One or more years’ experience working in a behavioral health setting preferred.

Previous experience with the patient population served by the facility preferred.Previous experience in case management preferred.

KEY FUNCTIONS :

  • Comprehensive daily housekeeping, support, and upkeep of physical plant and campus buildings
  • Responsible for the monitoring & maintenance of housekeeping equipment
  • On-call as needed for emergent housekeeping issues
  • Practices good safety techniques
  • Maintains a neat and orderly environment

EDUCATION / EXPERIENCE / SKILL REQUIREMENTS :

  • High school diploma or equivalent required.
  • At least one year of related experience, preferably in a hospital setting.

LICENSES / DESIGNATIONS / CERTIFICATIONS :

  • Greet PRS and assist them in completing the Intake / Registration paperwork.
  • Obtain Identification and Insurance Cards
  • Collect co-payments / co-insurance at time of service.
  • Log all payments on the Payment Collection’s spreadsheet.
  • Ensure all PRS have signed in
  • Assist in maintaining logs, manuals and other documentation, as required.
  • Scan financial documents into billing system.
  • Register new patients into registration system
  • Prepare charts for new intakes
  • Back up to scheduling, charge entry and medical record tasks
